超声清创联合银离子敷料治疗糖尿病足溃疡的临床疗效

摘　　要：目的 观察超声清创联合银离子敷料治疗糖尿病足溃疡的临床疗效。方法 90例糖尿病足溃疡患者随机分为超声清创组(A组)、银离子敷料组(B组)和超声清创联合银离子敷料组(C组),每组30例。连续治疗28d后,采用Bates-Jensen伤口评价工具(BWAT)评估治疗前及治疗后第7、14、21和28天的创面感染情况。结果 治疗后,B组与A组各时间点的BWAT评分相仿(P>0.05)。治疗后第14、21和28天,C组BWAT评分低于A组和B组(P<0.05)。创面感染方面,在BWAT中选出反映创面感染情况的6个条目(BWAT-I)进行评分。治疗后第7天,三组BWAT-I评分差异均无统计学意义(P>0.05);治疗后第28天,B组BWAT-I评分低于A组(P<0.05);治疗后第14、21、28天,C组BWAT-I评分低于A组和B组(P<0.05)。结论 联合使用超声清创与银离子敷料治疗糖尿病足溃疡,可以有效减轻并控制创面感染,促进创面愈合。Objective To observe the clinical efficacy of ultrasonic debridement combined with silver ion dressing in the treatment of diabetic foot ulcer.Methods A total of 90 patients with diabetic foot ulcer were randomly divided into three groups with 30 cases each,who were treated with ultrasonic debridement(group A),silver ion dressing(group B)or ultrasonic debridement combined with silver ion dressing(group C)for continuous 28 days.The clinical efficacy of the treatments was evaluated with Bates-Jensen Wound Assessment Tool(BWAT)score on the 7th,14th,21st and 28h day after treatment.Results After treatment,there were no significant differences in BWAT score of wound situation between group B and group A at each time point(P>0.05).On the 14th,21st and 28th day,the BWAT score of group C was significantly lower than that of group A and group B(P<0.05).In terms of wound infection,six items reflecting the situation of wound infection were selected in BWAT(BWAT-I)for scoring.There was no significant difference in BWAT-I score among the three groups on the 7th day(P>0.05).On the 28th day,the BWAT-I score of group B was lower than that of group A(P<0.05).On the 14th,21st and 28th day,the BWAT-I score of group C was significantly lower than that of group A and group B(P<o.o5).Conclusion In the treatment of diabetic foot ulcer,combined use of ultrasonic debridement and silver ion dressing can more effectively reduce and control wound infections and promote the wound healing.
