摘　　要：目的构建急性缺血性卒中(acute ischemic stroke,AIS)溶栓患者早期神经功能恶化(early neurological deterioration,END)护理风险矩阵模型。方法于2022年9—11月基于文献研究与分析、德尔菲专家咨询法、问卷调查法和风险矩阵理论构建AIS溶栓患者END护理风险矩阵,并用Borda序值法完善原始风险矩阵。结果构建的AIS溶栓患者END护理风险矩阵模型共有5个维度,51个风险条目中,极高风险16个条目(31.4%),高风险条目29个(56.9%),中风险条目6个(11.7%),无低风险条目。结论本研究构建的护理风险矩阵模型可为临床护士提供科学、直观的风险防范重点,并显著降低AIS溶栓患者END的风险。Objective To construct a nursing risk matrix model of early neurological deterioration(END)in thrombolytic patients with acute ischemic stroke(AIS).Methods The END nursing risk matrix of AIS thrombolytic patients was constructed based on literature research and analysis,Delphi expert consultation method,questionnaire survey,and risk matrix theory,and the original risk matrix was improved by Borda sequence value method from September to November 2022.Results There were 5 dimensions in the END care risk matrix model for AIS thrombolytic patients,and among 51 risk items,16 items were very high risk(31.4%),29 items were high risk(56.9%),6 items were medium risk(11.7%),and no items were low risk.Conclusion The nursing risk matrix model constructed in this study can provide clinical nurses with scientific and intuitive risk prevention priorities,and significantly reduce the risk of END in AIS thrombolytic patients.
