摘　　要：目的探讨早期糖尿病肾病(DKD)的主要影响因素,为早期识别与预防DKD提供循证依据。方法计算机检索The Cochrane Library、Web of Science、PubMed、中国知网和万方数据库中关于早期DKD的相关文献,检索时限为2009年1月1日至2023年12月31日。采用Stata 17.0软件进行统计分析。结果共纳入13篇文献,病例共5065例,包括合并早期DKD患者1405例。Meta分析结果显示,早期DKD的影响因素包括病程(OR=1.088,95%CI:1.029~1.150,P=0.003)、体质量指数(BMI,OR=1.097,95%CI:1.068~1.126,P<0.001)、低密度脂蛋白胆固醇(LDL-C,OR=2.176,95%CI:1.760~2.692,P<0.001)、空腹血糖(FBG,OR=1.410,95%CI:1.045~1.902,P=0.024)、收缩压(SBP,OR=1.014,95%CI:1.008~1.020,P<0.001)、糖化血红蛋白(HbA1c,OR=1.681,95%CI:1.298~2.177,P<0.001)、左心室后壁厚度(LVPW,OR=2.773,95%CI:1.472~5.226,P=0.002)、室间隔厚度(IVS,OR=3.278,95%CI:1.679~6.401,P=0.001)。结论病程、BMI、LDL-C、FBG、SBP、HbA1c、LVPW、IVS均为早期DKD的影响因素,该研究可为临床预警早期DKD提供循证依据。Objective To explore the main influencing factors for early diabetic kidney disease(DKD)and provide evidence-based support for the early identification and prevention of DKD.Methods A computer-based literature search was conducted in The Cochrane Library,Web of Science,PubMed,China National Knowledge Infrastructure(CNKI)and Wanfang databases for studies related to early DKD,with the search period from January 12009 to December 312023.Statistical analysis was performed using Stata 17.0 software.Results A total of 13 studies were included,with a total of 5065 cases,including 1405 patients with early DKD.The Meta-analysis results showed that the influencing factors for early DKD included disease duration(OR=1.088,95%CI:1.029-1.150,P=0.003),body mass index(BMI,OR=1.097,95%CI:1.068-1.126,P<0.001),low-density lipoprotein cholesterol(LDL-C,OR=2.176,95%CI:1.760-2.692,P<0.001),fasting blood glucose(FBG,OR=1.410,95%CI:1.045-1.902,P=0.024),systolic blood pressure(SBP,OR=1.014,95%CI:1.008-1.020,P<0.001),hemoglobin A1c(HbA1c,OR=1.681,95%CI:1.298-2.177,P<0.001),left ventricular posterior wall thickness(LVPW,OR=2.773,95%CI:1.472-5.226,P=0.002)and interventricular septum thickness(IVS,OR=3.278,95%CI:1.679-6.401,P=0.001).Conclusion Disease duration,BMI,LDL-C,FBG,SBP,HbA1c,LVPW and IVS are all influencing factors for early DKD.This study provides evidence-based support for clinical early warning and prevention of DKD.
