慢性心力衰竭患者血清爱帕琳肽、胎盘生长因子、脑啡肽酶水平与左心室重构的关系研究

摘　　要：目的探讨慢性心力衰竭(CHF)患者血清爱帕琳肽、胎盘生长因子(PLGF)、脑啡肽酶(NEP)水平与左心室重构的关系。方法选取2022年1月至2023年11月在哈尔滨市第二医院确诊的106例CHF患者作为CHF组,根据美国纽约心脏病协会(NYHA)心功能分级标准将CHF组分为Ⅱ级组(42例)、Ⅲ级组(34例)、Ⅳ级组(30例),另随机选取同期60例健康体检者为对照组。CHF组和对照组均接受血清学检查和超声心动图检查,比较2组血清学指标(爱帕琳肽、PLGF、NEP)水平,左心室重构相关指标[左心室舒张末期内径(LVEDD)、左心房前后径(LAAPD)、左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)、左心室射血分数(LVEF)]。采用Pearson相关分析血清爱帕琳肽、PLGF、NEP水平与左心室重构指标的相关性。结果CHF组血清爱帕琳肽、NEP水平低于对照组(P<0.05),PLGF水平高于对照组(P<0.05);CHF组LVEDD、LAAPD、LVEDV、LVESV高于对照组(P<0.05),LVEF低于对照组(P<0.05)。Ⅳ级组血清爱帕琳肽、NEP水平低于Ⅱ级组、Ⅲ级组,且Ⅲ级组低于Ⅱ级组,差异均有统计学意义(P<0.05);Ⅳ级组PLGF水平高于Ⅱ级组、Ⅲ级组,且Ⅲ级组高于Ⅱ级组,差异均有统计学意义(P<0.05)。Ⅳ级组LVEDD、LAAPD、LVEDV、LVESV高于Ⅱ级组、Ⅲ级组,且Ⅲ级组高于Ⅱ级组,差异均有统计学意义(P<0.05),Ⅳ级组LVEF低于Ⅱ级组、Ⅲ级组,且Ⅲ级组低于Ⅱ级组,差异均有统计学意义(P<0.05)。Pearson相关分析结果显示,CHF组患者爱帕琳肽、NEP水平与LVEDD、LAAPD、LVEDV、LVESV均呈负相关(P<0.05),与LVEF呈正相关(P<0.05);PLGF水平与LVEDD、LAAPD、LVEDV、LVESV均呈正相关(P<0.05),与LVEF呈负相关(P<0.05)。结论CHF患者血清爱帕琳肽、PLGF、NEP水平与左心室重构指标均存在相关性,可用于临床评估病情进展。Objective To investigate the relationship between serum apelin,placental growth factor(PLGF),neprilysin(NEP)levels and left ventricular remodeling in patients with chronic heart failure(CHF).Methods A total of 106 patients with CHF diagnosed at the Second Hospital of Harbin from January 2022 to November 2023 were selected for the CHF group.According to the New York Heart Association(NYHA)functional classification,the CHF group was divided into the gradeⅡgroup(42 cases),the gradeⅢgroup(34 cases),and the gradeⅣgroup(30 cases).A total of 60 healthy individuals who underwent physical examinations during the same period were randomly selected as the control group.Both the CHF group and the control group underwent serological testing and transthoracic echocardiography.The levels of serum apelin,PLGF and NEP,as well as left ventricular remodeling-related indices[left ventricular end-diastolic diameter(LVEDD),left atrial anteroposterior diameter(LAAPD),left ventricular end-diastolic volume(LVEDV),left ventricular end-systolic volume(LVESV)and left ventricular ejection fraction(LVEF)]were compared between the two groups.Pearson correlation analysis was used to assess the correlation between serum apelin,PLGF and NEP levels and the left ventricular remodeling indices.Results The serum levels of apelin and NEP in the CHF group were lower than those in the control group(P<0.05),while the PLGF level was higher than that in the control group(P<0.05).Compared with the control group,the LVEDD,LAAPD,LVEDV and LVESV in the CHF group were higher(P<0.05),and the LVEF was lower(P<0.05).The serum levels of apelin and NEP in the gradeⅣgroup were lower than those in the gradeⅡgroup and the gradeⅢgroup,and the levels of which in the gradeⅢgroup were lower than those in the gradeⅡgroup,and all differences were statistically significant(P<0.05).The PLGF level in the gradeⅣgroup was higher than that in the gradeⅡgroup and the gradeⅢgroup,and the level in the gradeⅢgroup was higher than that in the gradeⅡgroup,with
