摘　　要：目的:分析2023年山西省带状疱疹相关神经痛综合诊疗基层适宜性技术的推广成效。方法:本研究为前瞻性单臂试验。汇总2023年山西省带状疱疹相关神经痛综合诊疗基层适宜性技术的推广工作情况,其中,7家重点推广医院疼痛科的47名医护人员参与了问卷调查,问卷内容包括项目的适宜性、需求度、师资和推广内容的满意度、学习收获和意愿、技术掌握和应用情况及进一步的需求。比较推广前后各医院疼痛科带状疱疹相关神经痛患者月均门诊量、住院量及相应的医疗收入。使用SPSS 22.0进行统计学分析。结果:2023年山西省参加推广培训的医护人员527人,培养骨干医生32人,理论课66学时,义诊患者116人次、会诊疑难病例27人次。问卷调查显示本项目适宜性强,培训满意度高,并且能够切实提高患者疗效。各单位推广后疼痛科带状疱疹相关神经痛患者的月均门诊量[(5.9±6.2)人次/月比(14.2±11.2)人次/月, t=-3.41, P=0.014]、住院量[(4.2±4.4)人次/月比(9.6±7.0)人次/月, t=-4.55, P=0.004]及相应的医疗收入[(3.4±4.2)万元/月比(9.2±8.4)万元/月, t=-3.16, P=0.020]均升高,差异均有统计学意义。 结论:2023年山西省带状疱疹相关神经痛综合诊疗适宜性技术推广工作取得了明显成效。Objective To analyze the promoted effectiveness of the appropriate technology for comprehensive diagnosis and treatment of herpes zoster related neuralgia in primary healthcare of Shanxi Province in 2023.Methods This study was a prospective single arm trial.The promotion of appropriate technology for comprehensive diagnosis and treatment of herpes zoster related neuralgia in primary healthcare of Shanxi Province in 2023 was summarized,from the Department of Painology in 7 key cooperative hospitals,with 47 medical staff participated in the questionnaire.The questionnaire included the appropriateness of the project,the degree of demand,satisfaction with the faculty and promotion content,learning gains and willingness,mastery and application of technology,and further needs.The monthly average outpatient,inpatient volume and corresponding medical income of patients with herpes zoster related neuralgia were compared before and after the promotion.Statistical analysis was performed by SPSS 22.0.Results In 2023,527 medical staff participated in the promotion training in Shanxi Province,32 key doctors were trained,with 66 hours of theoretical courses,116 outpatient visits,and 27 consultations.The questionnaire survey showed that the project has strong suitability,high satisfaction with the training,and can effectively improve the patients'outcomes.After the promotion,the monthly average outpatient volume[(5.9±6.2)person-times/month vs.(14.2±11.2)person-times/month,t=-3.41,P=0.014],inpatient volume[(4.2±4.4)person-times/month vs.(9.6±7.0)person-times/month,t=-4.55,P=0.004],and corresponding medical income[(3.4±4.2)10 thousands CNY/month vs.(9.2±8.4)10 thousands CNY/month,t=-3.16,P=0.020]were significantly increased.Conclusion In 2023,the promotion of the primary appropriate technology for the comprehensive diagnosis and treatment of herpes zoster related neuralgia in Shanxi Province has achieved significant effectiveness.
