老年帕金森病衰弱现状及其与跌倒的相关性研究

摘　　要：目的以城市三甲医院为背景,调查老年帕金森病(parkinson's disease,PD)患者衰弱情况,研究衰弱与老年PD患者跌倒之间的相关性。方法选取2020年5月-2021年6月在江苏省苏北人民医院神经内科门诊就诊及住院的193例老年PD患者为研究对象,对所有患者进行人口学调查及健康状况调查,采用Hoehn-Yahr(H-Y)分期对老年PD患者病情严重程度进行评估;使用Fried衰弱症状学量表评估患者衰弱情况;采用跌倒情况调查表评估患者近一年跌倒情况,包括跌倒史和跌倒伤害分级;根据有无跌倒将入组老年PD患者分为非跌倒组和跌倒组,分析衰弱对老年PD患者跌倒的影响。结果共收集老年PD患者193例,其中衰弱者有77例(39.9%),近一年有跌倒史者95例(49.2%);单因素分析显示,年龄、服药数量、合并疾病种数、H-Y分期及衰弱分期是老年PD患者跌倒的高危因素;Logistic回归分析显示,衰弱、年龄、BMI、服药数量及合并疾病种数是老年PD患者跌倒的独立危险因素。结论老年PD患者衰弱和跌倒的发生率较高,且衰弱是老年PD患者跌倒的危险因素,应重视衰弱的早期识别与干预,进而为老年PD患者跌倒预防护理提供临床干预可靠依据。Objective This study investigated the asthenia status of patients with Parkinson disease(PD)among the elderly,and the correlation between asthenia and PD in the urban tertiary hospital.Methods From May 2020 to June 2021,the 192 elderly inpatients and outpatients in our hospital with PD were selected as research objects.Demographic survey and health survey were conducted and the asthenia of patients was evaluated by Fried Asthenic Symptomatology Scale.This study used falls questionnaire to analyse the patient's fall in the past year,including the history of fall and fall injury grading.We divided all the patients into two groups depending on whether they fell or not,in order to evaluate the effects which asthenia had on the fall of elderly patients with PD.Results A total of 193 elderly patients with PD were collected,of which 77 patients(39.9%)were asthenic,and 95 patients with a history of fall in the past year(49.2%).Single factors analysis showed that age,number of medication,the number of illnesses,H-Y staging,asthenia staging were risk factors for the fall of elderly PD patients.The logistic regression showed that asthenia,age,BMI,number of medicines,and the number of diseases were independent risks factors for elderly PD patients.Conclusion The incidence of fall in elderly PD patients with asthenia is high,and asthenia is the risk factor for fall in those people.Early identification and intervention of asthenia should be concerned so as to provide reliable evidence for fall prevention care in clinic prevention.
