基于健康人文视角的社区综合养老服务模式改进路径探析

摘　　要：目的了解北京市社区综合养老服务模式运行现状及发展困境,阐释健康人文理念对于推进社区综合养老服务模式有序发展的价值引领作用。方法采用质性研究方法,于2021年7月-2022年1月,对北京市4个区的8家社区养老服务机构的9名管理人员及机构服务半径覆盖范围内的127名社区老年居民进行半结构化的个人深入访谈,采用主题框架法对社区综合养老服务供需现状及现存困境进行定性资料分析。结果共提炼出4个主题:社区综合养老服务有区域差异,存在供需失衡现象;社区医养机构之间联动不足,社区综合养老服务衔接不畅;社区综合养老服务冲击传统家庭养老模式,造成家庭成员养老责任意识衰减;社区综合养老服务体系中老年人“主体性”缺失,主动意识较弱。结论基于健康人文理念,政府应消弭区域间社区综合养老服务差异,完善社区养老服务供需体制;社区应加强“医-养-社”三方联动,构建社区综合养老服务多元主体协作机制;家庭成员应切实履行家庭养老职责,推动家庭成员孝道文化传承;老年人应发挥主体性原则,激活自身的社区治理力量。Objective To understand the current status and development difficulties of Beijing's community comprehensive elderly care service model,and explain the value-leading role of healthy humanistic concepts in promoting the orderly development of community comprehensive elderly care service models.Methods Using qualitative research method,eight community elderly service organizations in four districts of Beijing were selected as the research sites,nine heads of community elderly service organizations and 127 elderly community residents were personally interviewed in-depth from July 2021 to January 2022.After the interview,this study used the thematic framework method to analyze the qualitative data of the current situation of supply and demand for community integrated aged care services and existing dilemmas.Results Four themes were extracted from this study:there are regional differences and imbalance in supply and demand in the process of providing community integrated aged care services;the insufficient linkage between community medical and health care institutions and poor connection of community integrated aged care services;the impact of community integrated aged care services on the traditional family elderly care model,resulting in the decay of family members'awareness of elderly care responsibilities;and the lack of subjectivity and weak initiative of elderly people in the community integrated aged care service system.Conclusion Based on the concept of health humanities,the government should eliminate the regional differences in integrated community elderly care services and improve the supply and demand system of community integrated aged care;the community should strengthen the tripartite linkage of"medical-nursing-community"and build a collaborative mechanism of multiple subjects;family members should fulfill the responsibility of family elderly care and promote the transmission of filial culture among family members;the elderly should play the principle of subjectivity and activate their own community gov
