摘  要:目的分析机器人引导下股骨粗隆间骨折复位防旋股骨近端髓内钉(PFNA)内固定的临床应用效果。方法选取2022年2月—2024年4月于云浮市人民医院进行手术的60例股骨粗隆间骨折患者为研究对象,根据术中是否使用骨科创伤机器人引导将研究对象分为机器引导组(n=30)与人工透视组(n=30),对比两组手术一般资料(指标包括透视次数、手术时间、术时出血量、导针调整次数)、复位情况、髋关节功能。结果机器引导组透视次数、术时出血量、导针调整次数明显低于人工透视组,手术时间明显短于人工透视组(P<0.05);术后机器引导组复位情况与髋关节功能明显优于人工透视组(P<0.05)。结论将骨科创伤机器人运用于骨粗隆间骨折复位PFNA内固定手术中效果确切,能够减少术时透视次数与导针调整次数,缩短手术时间,减少术时出血量,优化复位效果,增强髋关节功能。Objective To analyze clinical application effect of robot guided reduction of intertrochanteric fractures with anti rotation proximal femoral nail(PFNA)internal fixation.Methods The paper chose 60 patients with intertrochanteric fractures of the femur in Yunfu People's Hospital from February 2022 to April 2024 as study subjects,and divided them into the machine guided group(n=30)and the artificial fluoroscopy group(n=30)based on using orthopedic trauma robots for guidance during surgery or not.General surgical data were compared between two groups,including fluoroscopy frequency,surgical time,intraoperative blood loss,number of needle adjustments,reduction status and hip joint function.Results The machine guided group had significantly lower fluoroscopy frequency,intraoperative blood loss,and guide needle adjustment frequency than the manual fluoroscopy group,the surgical time was significantly shorter than the manual fluoroscopy group(P<0.05).The postoperative machine guided group showed significantly better reduction and hip joint function than the manual fluoroscopy group(P<0.05).Conclusion Orthopedic trauma robots can achieve definite effect in PFNA internal fixation surgery for reduction of intertrochanteric fractures,can reduce number of fluoroscopy and guide needle adjustments during surgery,shorten operation time,reduce bleeding amount during surgery,optimize the reduction effect,and enhance hip joint function of patients.
