老年男性2型糖尿病患者尿酸排泄分数与骨质疏松症的相关性分析

摘　　要：目的探究老年男性2型糖尿病患者尿酸排泄分数(FEUA)与骨质疏松症(OP)的相关性。方法依据骨密度(BMD)对2022年1月—2023年12月在连云港市赣榆区人民医院进行治疗的135例老年男性2型糖尿病患者进行分组,即A组(骨量正常)、B组(骨量减少)、C组(骨质疏松),每组45例。对各组均测定FEUA水平,分析FEUA和腰椎L1-4、股骨颈、髋关节的BMD的相关性。分析FEUA对OP的预测效能,通过绘制受试者工作特征(ROC)曲线,基于曲线下面积(AUC)进行评估。结果和A组比较,B组以及C组FEUA水平较高(P<0.05)。和B组比较,C组FEUA水平较高(P<0.05)。FEUA水平和腰椎L1-4、股骨颈、Wards三角以及大粗隆的BMD呈现负相关性(r=-0.687、-0.751、-0.789、-0.765,P<0.05)。分析ROC曲线,AUC为0.75,Youden指数最大对应的FEUA数值为12.29%。结论在老年男性2型糖尿病患者中,随着BMD的减少,FEUA会升高,FEUA和OP呈负相关。当FEUA数值高于12.29%时,对OP有一定预测价值。Objective To explore correlation between fraction excretion of uric acid(FEUA)and osteoporosis(OP)of elderly male patients with type 2 diabetes.Methods The paper chose 135 elderly male patients with type 2 diabetes in Lianyungang City Ganyu District People's Hospital from January 2022 to December 2023 according to the bone mineral density(BMD),and divided them into group A(with normal bone mass),group B(with osteopenia),and group C(with osteoporosis),with 45 cases in each group.FEUA levels in each group were detected,correlation between FEUA and BMD of lumbar vertebrae L1-4,femoral neck,and hip joint was analyzed.Predictive performance of FEUA for OP was analyzed by plotting receiver operating characteristic(ROC)curves,and area under the curve(AUC)was evaluated.Difference in FEUA levels among three groups was statistically significant(P<0.05).Group B and C had higher levels of FEUA than group A,difference was statistically significant(P<0.05).Group C had higher levels of FEUA than group B,difference was statistically significant(P<0.05).There was negative correlation between FEUA levels and BMD of lumbar vertebrae L1-4,femoral neck,Wards triangle,and greater trochanter(r=-0.687,-0.751,-0.789,-0.765,P<0.05).ROC curve analysis showed AUC was 0.75,and maximum Youden index corresponds to FEUA value was 12.29%.Conclusion FEUA of of elderly male patients with type 2 diabetes,may increase with decrease of BMD,and FEUA is negatively correlated with OP.FEUA value has some certain predictive value for OP when it is higher than 12.29%.
