血液灌流、血液透析联合治疗急性有机磷中毒的疗效及对患者心肌功能的影响

摘　　要：目的研究血液灌流、血液透析联合治疗急性有机磷中毒(AOPIP)的临床效果。方法回顾性选择本院收治的60例AOPIP患者为研究对象,根据治疗方式的不同将其分为XT组(30例)和XT+XG组(30例)。其中,XT组患者单纯应用血液透析治疗,XT+XG组患者应用血液灌流、血液透析联合治疗。对比两组患者的救治效果,并于治疗前后分别检测患者血清炎症反应指标(sST2、NF-κB)、血清心肌损伤指标(cTnT、CK-MB、NT-pro BNP)。结果XT+XG组患者救治成功率高于XT组(P<0.05)。XT+XG组患者ChE恢复时间短于XT组(P<0.05)。治疗后,XT+XG组患者血清sST2、NF-κB和cTnT、CK-MB、Pro-BNP均低于XT组(P<0.05)。结论血液净化联合血液灌流治疗可有效提高AOPIP患者的救治效果,提高患者炎性介质清除效果,减轻心肌损伤程度。Objective To investigate clinical effect of combination of hemoperfusion and hemodialysis in treatment of acute organophosphate poisoning(AOPIP).Methods The paper reviewed and chose 60 AOPIP patients in our hospital,and divided them into XT group(30 cases)and XT+XG group(30 cases)according to different treatment methods.The XT group was treated with hemodialysis only,while the XT+XG group with hemoperfusion combined with hemodialysis.Treatment outcomes were compared between two groups,serum inflammatory response indicators(sST2,NF-κB)and serum myocardial injury indicators(cTnT CK-MB、NT-pro BNP)were detected before and after treatment.Results Success rate of treatment in the XT+XG group was higher than the XT group(P<0.05).The ChE recovery time in the XT+XG group was shorter than the XT group(P<0.05).After treatment,serum levels of sST2,NF-κB,cTnT,CK-MB,and Pro BNP in the XT+XG group were lower than the XT group(P<0.05).Conclusion Combination of hemoperfusion and hemodialysis therapy can improve treatment effect of AOPIP patients effectively,enhance clearance of inflammatory mediators,and reduce degree of myocardial injury.
