侧小切口与后路小切口髋关节置换术治疗老年创伤性股骨颈骨折的临床效果

摘　　要：目的解析在老年创伤性股骨颈骨折治疗上选择侧小切口和后路小切口髋关节置换术的应用效果。方法选取2023年3月—2024年5月本院收治的50例接受手术治疗的老年创伤性股骨颈骨折患者,以随机抽签方式将其平分为两组,对照组(25例)进行侧小切口髋关节置换术,观察组(25例)则实施后路小切口髋关节置换术,就两组的手术疗效、围手术指标、疼痛程度以及并发症等指标进行逐一对比。结果两组疗效均较高,数据差异无统计学意义(P>0.05);在手术时间上两组相差不大(P>0.05),观察组术中失血量及术后引流量上较对照组更少(P<0.05);治疗前,两组疼痛差异不大(P>0.05),治疗后,观察组疼痛分值较对照组低(P<0.05);两组在并发症方面的差异无统计学意义(P>0.05)。结论在老年创伤性股骨颈骨折治疗上选择侧小切口和后路小切口髋关节置换术均具良好疗效和安全性,但后者在失血量、引流量和疼痛感上效果更高,更具应用与推广价值。Objective To analyze application value of lateral small incision and posterior small incision for traumatic femoral neck fractures of elderly patients with hip replacement surgery.Methods The paper chose 50 elderly patients with traumatic femoral neck fractures with surgical treatment in our hospital from March 2023 to May 2024,and divided them into two groups randomly by drawing lots.The control group(25 cases)was treated with lateral small incision hip replacement surgery,while the observation group(25 cases)with posterior small incision hip replacement surgery.Surgical effect,perioperative indicators,pain level,and complications were compared between two groups.Results Both groups had high curative effect,data difference was not statistically significant(P>0.05).Difference in surgical time between two groups was not significant(P>0.05),the observation group had less intraoperative blood loss and postoperative drainage than the control group(P<0.05).There was no significant difference in pain between two groups before treatment(P>0.05).After treatment,the observation group had lower pain scores than the control group(P<0.05).There was no statistically significant difference in complications between two groups(P>0.05).Conclusion Both lateral and posterior small incision hip arthroplasty can achieve good effect and safety for traumatic femoral neck fractures of elderly patients.However,the latter is more effective in terms of blood loss,drainage volume,and pain sensation,which has higher application and promotion value.
