除痹止痛汤联合温针灸治疗对寒湿痹阻型腰椎间盘突出症患者腰椎功能康复的影响

摘　　要：目的探讨除痹止痛汤联合温针灸治疗对寒湿痹阻型腰椎间盘突出症(LDH)患者的影响。方法选取2023年4月—2024年4月仙桃市第一人民医院收治的120例寒湿痹阻型LDH患者为研究对象,按随机数字表法将其分为对照组和观察组,每组60例。对照组患者予以温针灸治疗,观察组患者在对照组的基础上给予除痹止痛汤治疗,两组患者均治疗4周。比较两组的中医证候积分、腰椎功能、腰椎活动度、疼痛程度、不良反应发生情况。结果治疗后,观察组腿痛沉重、腰痛、肢体发凉、腿麻评分均低于对照组,日常生活能力、临床体征检查、自觉主观症状评分均高于对照组,侧屈、屈伸、旋转活动度均大于对照组,组间差异有统计学意义(P<0.05);观察组疼痛视觉模拟评分为(1.65±0.54)分,低于对照组的(3.98±0.78)分,差异有统计学意义(P<0.05)。两组患者均未出现明显不良反应。结论除痹止痛汤联合温针灸治疗寒湿痹阻型LDH患者的效果显著,可促进患者临床症状改善,缓解疼痛,改善患者的腰椎活动度及腰椎功能。Objective Explore the impact of the treatment of Chubi Zhitong Decoction combined with warm needle acupuncture on patients with lumbar disc herniation(LDH)of the cold-dampness obstruction type.Methods One hundred and twenty patients with LDH of the cold-dampness obstruction type who were admitted to Xiantao First People's Hospital from April 2023 to April 2024 were selected as the research objects.They were divided into a control group and an observation group according to the random number table method,with 60 cases in each group.Patients in the control group were treated with warm needle acupuncture,while patients in the observation group were given Chubi Zhitong Decoction on the basis of the treatment of the control group,both groups of patients were treated for four weeks.The Traditional Chinese Medicine syndrome scores,lumbar spine function,lumbar spine range of motion,pain level and the occurrence of adverse reactions of the two groups were compared.Results After treatment,the scores of heavy leg pain,low back pain,cold limbs and numbness in the legs in the observation group were all lower than those in the control group,the activities of daily living,the results of clinical physical examination and the subjective symptoms of self-awareness in the observation group were all higher than those in the control group,the ranges of motion in lateral flexion,flexion-extension and rotation in the observation group were all greater than those in the control group,the differences between the groups were statistically significant(P<0.05);the Visual Analogue Scale score for pain in the observation group was(1.65±0.54)points,which was lower than(3.98±0.78)points in the control group,and the difference was statistically significant(P<0.05).No obvious adverse reactions occurred in the patients of both groups.Conclusion The treatment of Chubi Zhitong Decoction combined with warm needle acupuncture on the LDH of the cold-dampness obstruction type has a remarkable effect.It can promote the improvement of patients'clinical
