中药膏摩联合Mulligan动态关节松动术治疗肩袖损伤的临床疗效观察

摘　　要：目的探讨中药膏摩联合Mulligan动态关节松动术(MWM)治疗肩袖损伤的临床疗效。方法选取北京市昌平区中西医结合医院2021年12月—2023年7月收治的90例肩袖损伤患者为研究对象,采用随机数字表法将其分为中药膏摩组、康复组和中西结合组,各30例。中西结合组采用中药膏摩联合MWM治疗,中药膏摩组采用中药膏摩治疗,康复组采用MWM治疗,三组均治疗6周。比较三组的临床疗效、疼痛程度、肩关节功能及肩关节活动度。结果中西结合组治疗总有效率高于康复组、中药膏摩组,差异有统计学意义(P<0.05)。治疗后,三组疼痛视觉模拟(VAS)评分、美国加州大学肩关节评分(UCLA)比较,组间差异有统计学意义(P<0.05);进一步两两比较结果显示,中西结合组VAS评分低于康复组、中药膏摩组,且中药膏摩组低于康复组,UCLA评分高于康复组、中药膏摩组,且康复组高于中药膏摩组,组间差异有统计学意义(P<0.05);三组肩关节活动度比较,组间差异有统计学意义(P<0.05);进一步两两比较结果显示,中西结合组肩关节活动度大于康复组、中药膏摩组,且康复组大于中药膏摩组,组间差异有统计学意义(P<0.05)。结论中药膏膜联合MWM治疗肩袖损伤的效果较好,有利于减轻疼痛,促进肩关节功能改善,值得推广。Objective To investigate the clinical efficacy of traditional Chinese medicine ointment and massage combined with Mulligan mobilization with movement(MWM)in the treatment of rotator cuff injury.Methods A total of 90 patients with rotator cuff injuries admitted to Changping District Hospital of Traditional Chinese and Western Medicine in Beijing from December 2021 to July 2023 were selected as the research subjects.They were randomly divided into three groups using a random number table:traditional Chinese medicine ointment and massage group,rehabilitation group,and integrated Chinese and Western medicine group,with 30 cases in each group.The integrative medicine group was treated with traditional Chinese medicine ointment and massage combined with MWM,the traditional Chinese medicine ointment and massage group was treated with traditional Chinese medicine ointment and massage,and the rehabilitation group was treated with MWM,all three groups were treated for 6 weeks.The clinical efficacy,degree of pain,shoulder joint function and shoulder range of motion of the three groups were compared.Results The total effective rate of the combination of Chinese and Western medicine treatment group was higher than that of the rehabilitation group and the traditional Chinese medicine ointment and massage group,and the differences were statistically significant(P<0.05).After treatment,there were statistically significant differences in the pain Visual Analogue Scale(VAS)scores and University of Couifornia of Los Angeles(UCLA)scores among the three groups(P<0.05);further pairwise comparison results showed that the VAS score of the integrated Chinese and Western medicine group was lower than that of the rehabilitation group and the traditional Chinese medicine ointment and massage group,and the traditional Chinese medicine ointment and massage group was lower than the rehabilitation group,and the UCLA score was higher than that of the rehabilitation group and the traditional Chinese medicine ointment and massage group, and the re
