桃红四物汤加味联合温针灸治疗对肩袖损伤患者术后炎症指标及疼痛程度的影响

摘　　要：目的探讨桃红四物汤加味联合温针灸治疗对肩袖损伤患者的影响。方法选取烟台市蓬莱区中医医院2022年5月—2024年5月收治的82例肩袖损伤患者为研究对象,按随机数字表法将其分为对照组和观察组,各41例。对照组采用温针灸治疗,观察组在对照组基础上采用桃红四物汤加味治疗,两组均持续治疗12周。比较两组的肩关节功能、炎症指标、疼痛程度、肩关节活动度。结果治疗后,两组美国肩与肘协会评分系统、美国加州大学肩关节评分系统评分均高于治疗前,且观察组均高于对照组,差异有统计学意义(P<0.05);两组白细胞介素-6、肿瘤坏死因子-α水平均低于治疗前,且观察组均低于对照组,差异有统计学意义(P<0.05);观察组疼痛视觉模拟评分低于对照组,肩关节活动度中前屈、后伸、内旋及外旋均大于对照组,组间差异有统计学意义(P<0.05)。结论桃红四物汤加味联合温针灸治疗可缓解肩袖损伤患者的疼痛程度,加快炎症消退,可改善其肩关节功能。Objective To explore the effect of modified Taohong Siwu Decoction combined with warm acupuncture and moxibustion on patients with rotator cuff injury.Methods A total of 82 patients with rotator cuff injuries admitted to Penglai District Traditional Chinese Medicine Hospital in Yantai City from May 2022 to May 2024 were selected as the research subjects,according to the random number table method,they were divided into a control group and an observation group,with 41 cases in each group.The control group was treated with warm acupuncture and moxibustion,and the observation group was treated with modified Taohong Siwu Decoction on the basis of the control group,both groups were treated continuously for 12 weeks.Compare the shoulder joint function,inflammation indicators,pain degree,and shoulder joint range of motion between two groups.Results After treatment,the scores of the American Shoulder and Elbow Scoring System and the University of California at Los Angeles in both groups were higher than those before treatment,and the observation group were higher than those of the control group,the differences were statistically significant(P<0.05);the levels of interleukin-6 and tumor necrosis factor alpha in both groups were lower than those before treatment,and the observation group were lower than those of the control group,the differences were statistically significant(P<0.05);the scores of the pain Visual Analogue Scale of the observation group scored lower than that of the control group,and the range of motion of the shoulder joint,including flexion,extension,internal rotation,and external rotation,were all greater than those of the control group,the differences between the groups were statistically significant(P<0.05).Conclusion Modified Taohong Siwu Decoction combined with warm acupuncture and moxibustion can alleviate the pain degree of patients with rotator cuff injury,accelerate the regression of inflammation,and improve the shoulder joint function.
