摘　　要：目的分析特发性语言发育迟缓患儿脑电特征与治疗效果的关系,为言语障碍的治疗提供参考依据。方法选取2023年1月—2024年2月江门市妇幼保健院收治的66名特发性语言发育迟缓患儿为研究对象,对患儿进行视频脑电图(VEEG)、0~6岁小儿神经心理发育检查表、S-S法语言发育迟缓评定量表检查。根据VEEG结果分为正常VEEG组(正常组)和异常VEEG组(异常组),分析其脑电特征与发育商的关系;对所有患儿进行语言训练,6个月后对比两组患儿的治疗效果。结果66例特发性语言发育迟缓患儿中,VEEG结果正常47例(71.21%),异常19例(28.79%),其中痫样放电5例(7.57%),背景异常14例(21.21%)。背景异常是特发性语言发育迟缓患儿VEEG异常的主要表现。中重度特发性语言发育迟缓患儿VEEG异常率高于轻度特发性语言发育迟缓患儿,差异有统计学意义(P<0.05)。正常组治疗总有效率为76.60%,高于异常组的47.37%,差异有统计学意义(P<0.05)。结论言语障碍程度越严重,脑功能障碍越突出。VEEG对判断言语障碍的严重程度及推测预后方面均有优势。Objective To analyze the relationship between EEG characteristics and therapeutic effect in children with idiopathic speech retardation,and to provide reference for the treatment of speech disorders.Methods A total of 66 children with idiopathic language retardation admitted to the Jiangmen Women and Children's Health Hospital from January 2023 to February 2024 were selected as the research objects.The children were examined by video electroencephalogram(VEEG),neuropsychological development checklist for children aged 0 to 6 years old and S-S Language retardation Rating Scale.They were divided into a normal VEEG group(normal group)and an abnormal VEEG group(abnormal group)according to EEG results,and the relationship between EEG characteristics and developmental quotient was analyzed.All the children were given language training,and the therapeutic effect was compared between the two groups after 6 months.Results Among 66 children with idiopathic language delay,VEEG results were normal in 47 cases(71.21%)and abnormal in 19 cases(28.79%),including 5 cases(7.57%)of epileptiform discharges and 14 cases(21.21%)of background abnormalities.Background abnormality is the main manifestation of VEEG abnormality in children with idiopathic language retardation.The abnormal rate of VEEG in children with moderate and severe speech delay was higher than that in children with mild speech delay,and the difference was statistically significant(P<0.05).The total effective rate of the normal group was 76.60%,which was higher than 47.37%of the abnormal group,and the difference was statistically significant(P<0.05).Conclusion The more severe the speech disorder,the more prominent the brain dysfunction.VEEG has advantages in judging the severity of speech disorder and predicting prognosis.
