早期颈肩部锻炼在甲状腺手术患者快速康复外科护理中的应用分析

摘　　要：目的分析早期颈肩部锻炼在甲状腺手术患者快速康复外科(ERAS)护理中的应用效果。方法选取2020年10月—2023年9月新泰市人民医院接受甲状腺手术治疗的98例患者为研究对象,按随机数字表法将其分为对照组与研究组,各49例。对照组采用ERAS护理,研究组在对照组基础上采用早期颈肩部锻炼。比较两组的术后情况、颈部活动度、肩关节功能及术后并发症发生情况。结果两组术后24、48、72 d的引流量、住院时间对比,组间差异无统计学意义(P>0.05)。研究组颈部向患侧旋转、向健侧旋转、前屈与后伸角度均大于对照组,组间差异有统计学意义(P<0.05)。术后4周,研究组Constant-Murley肩关节评分量表评分高于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率为4.08%,低于对照组的18.37%,差异有统计学意义(P<0.05)。结论早期颈肩部锻炼在甲状腺手术患者ERAS护理中具有显著的应用效果,可以改善其颈部活动度,促进肩关节功能恢复,降低并发症风险。Objective To analyze the application effect of early neck and shoulder exercise in enhanced recovery after surgery(ERAS)nursing of patients with thyroid surgery.Methods A total of 98 patients who underwent thyroid surgery in Xintai People's Hospital from October 2020 to September 2023 were selected as the research objects,according to the random number table method,they were divided into a control group and a research group,with 49 cases in each group.The control group was given ERAS nursing,and the research group was given early neck and shoulder exercise on the basis of the control group.The postoperative conditions,neck range of motion,shoulder joint function and postoperative complications were compared between the two groups.Results There was no significant difference in drainage volume and length of hospital stay between the two groups at 24,48 and 72 days after operation(P>0.05).The angles of neck rotation to the affected side,rotation to the healthy side,flexion and extension in the research group were larger than those in the control group,and the differences between the groups were statistically significant(P<0.05).Four weeks after operation,the Constant-Murley Shoulder Scale score of the research group was higher than that of the control group,and the difference was statistically significant(P<0.05).The incidence of postoperative complications in the research group was 4.08%,which was lower than 18.37%in the control group,and the difference was statistically significant(P<0.05).Conclusion Early neck and shoulder exercise has a significant application effect in ERAS nursing of patients undergoing thyroid surgery,which can improve their neck mobility,promote shoulder joint function recovery,and reduce the risk of complications.
