摘　　要：目的分析临床药师在心房颤动患者华法林抗凝治疗管理中的作用。方法选取2019年1—12月在柳州市工人医院接受华法林治疗的120例心房颤动患者为研究对象,根据是否有临床药师参与用药管理,分为对照组(无临床药师参与用药管理)与管理组(有临床药师参与用药管理)各60例。比较两组患者用药依从性和国际标准化比值(INR)监测依从性,INR达标率以及血栓和出血事件发生情况。结果管理组研究对象用药依从性、INR监测依从性高于对照组(P<0.05);治疗1年后,管理组研究对象INR达标率高于对照组(P<0.05);治疗期间,两组血栓和出血事件发生情况差异无统计学意义(P>0.05)。结论对于采取华法林进行抗凝治疗的心房颤动患者,由临床药师参与用药的管理和监护,可提高患者用药和INR监测的依从性,改善抗凝治疗的达标率,安全性可靠。Objective To analyze the role of clinical pharmacists in the management of Warfarin anticoagulation therapy in patients with atrial fibrillation.Methods A total of 120 patients with atrial fibrillation who received Warfarin treatment in Liuzhou Workers'Hospital from January to December 2019 were selected as the study subjects,and they were divided into control group(no clinical pharmacist involved in medication management)and management group(with clinical pharmacists involved in medication management),60 cases in each group.The medication compliance and international normalized ratio(INR)monitoring compliance,the INR compliance rate and the occurrence of thrombosis and bleeding events were compared between the two groups.Results The medication compliance and INR monitoring compliance of the subjects in the management group were higher than those in the control group(P<0.05);after 1 year of treatment,the INR compliance rate of the management group was higher than that of the control group(P<0.05);during the treatment period,there was no significant difference in the incidence of thrombotic and bleeding events between the two groups(P>0.05).Conclusions For patients with atrial fibrillation treated with Warfarin for anticoagulation,clinical pharmacists can improve the compliance of patients with medication and INR monitoring,improve the compliance rate of anticoagulation therapy,and the safety is reliable.
