摘 要:目的探讨联合超声技术在甲状腺结节良恶性鉴别中的应用价值。方法回顾性分析2022年6月—2024年1月柳州市工人医院400例甲状腺结节患者的临床资料。比较常规超声、超声弹性成像、超声造影、萤火虫技术、超声造影联合超声弹性成像对甲状腺结节的检出情况。结果病理检查结果证实,400例(514个结节)患者中,恶性结节占35.41%,包括髓样癌、滤泡癌、乳头状癌;良性结节占64.59%,包括腺瘤、结节性甲状腺肿、桥本结节;良性组和恶性组常规超声检查特征(包括结节回声、回声均匀性、形态、边缘、钙化情况、纵横比、后方回声衰减)、超声弹性成像评分比较,差异有统计学意义(P<0.05);萤火虫技术显示,良性结节呈粗大弧形及颗粒状钙化,恶性结节表现为微小沙粒样钙化,或呈簇状分布;超声造影联合超声弹性成像诊断甲状腺结节良恶性的准确性最高。结论联合超声技术对甲状腺结节良恶性的诊断准确性高,具有临床应用价值。Objective To investigate the application value of combined ultrasound technology in the differentiation of benign and malignant thyroid nodules.Methods The clinical data of 400 patients with thyroid nodules in Liuzhou Workers'Hospital from June 2022 to January 2024 were retrospectively analyzed.The detection of conventional ultrasound,contrast-enhanced ultrasound combined with ultrasound elastography,contrast-enhanced ultrasound alone,ultrasound elastography and firefly technology for thyroid nodules were compared.Results The results of pathological examination confirmed that among 400 patients(514 nodules),malignant nodules accounted for 35.41%including medullary carcinoma,follicular carcinoma and papillary carcinoma;benign nodules accounted for 64.59%,including adenoma,nodular goiter,hashimoto nodules;there were significant differences in conventional ultrasound features(including nodule echo,echo,shape,edge,calcification,aspect ratio,posterior attenuation)between benign and malignant groups,there was a statistically significant difference in ultrasound elastography scores(P<0.05);the firefly technique showed that benign nodules showed coarse arc-shaped and granular calcification,while malignant nodules showed small sand-like calcification or clustered distribution;the diagnostic accuracy of contrast-enhanced ultrasound combined with ultrasound elastography is the highest.Conclusions Combined ultrasound technology can show a good diagnostic effect in the differentiation of benign and malignant thyroid nodules,which is helpful to provide correct diagnosis in clinical practice.
