外科手术治疗肺结核合并肺曲菌球病的效果观察

摘　　要：目的观察肺结核合并肺曲菌球病患者采取外科手术治疗的效果。方法选取2021年1月—2022年1月贵阳市公共卫生救治中心收治的45例肺结核合并肺曲菌球病患者,均采取外科手术治疗,观察患者的治疗效果。结果35例孤立性病变病灶彻底切除;双肺多发性病变者9例,对空洞病灶采取肺叶切除术后结合抗真菌、抗结核治疗6个月及1年后病灶消失。将切除标本进行病理学诊断均明确为肺曲菌球病,33.33%明确为侵犯胸膜。无死亡病例,术后并发症发生率为15.16%。随访1年,所有患者未发生肺曲菌球病复发,且生活质量较好。结论肺结核合并肺曲菌球病患者术前诊断准确率不高,对其采取外科手术治疗,可获得显著的治疗效果。Objective To analyze the effect of surgical treatment on patients with pulmonary tuberculosis complicated with pulmonary aspergilloma.Methods A total of 45 patients with pulmonary tuberculosis complicated with pulmonary aspergilloma admitted to Guiyang Public Health Treatment Center from January 2021 to January 2022 were selected and all received surgical treatment,the treatment effects of the patients were observed.Results In 35 cases with solitary lesions,the lesions were completely removed;there were 9 cases with multiple bilateral pulmonary lesions,after lobectomy of the cavitary lesions combined with antifungal and anti-tuberculosis treatment for 6 months and 1 year,the lesions disappeared.The pathological diagnosis of the resected specimens all confirmed pulmonary aspergilloma,and 33.33%of the cases were confirmed to have invaded the pleura.There were no deaths,the incidence of postoperative complications was 15.16%.After 1 year of follow-up,no recurrence of pulmonary aspergilloma occurred,and the patients had a good quality of life.Conclusions The preoperative diagnostic accuracy of pulmonary tuberculosis complicated with pulmonary aspergilloma patients is not high,surgical treatment for patients can achieve significant treatment effects.
