摘 要:目的探讨在临床病原菌监测中实施细菌耐药性监测的临床价值。方法选取2023年1—11月甘肃省武威肿瘤医院各科室检出的117例病原菌感染患者的微生物标本,均行微生物检验和药敏试验,分析细菌耐药性的监测效果。结果117株多重耐药菌(MDRO)中,包括产ESBLs大肠埃希菌95株,耐药率最高为环丙沙星(77.89%);产ESBLs肺炎克雷伯菌12株,耐药率最高为磺胺甲恶唑(25.00%);耐甲氧西林金黄色葡萄球菌(MRSA)10株,耐药率最高为磺胺甲恶唑(30.00%)。2023年1—11月MDRO检出率最高为1月份和11月份,分别为29.73%和28.57%;117株MDRO中,尿液(27.35%)和痰液(17.09%)标本检出率最高;在科室分布上,泌尿外科占比最高,为13.68%;在年龄分布上,71~80岁年龄段检出率较高(37.50%)。结论在临床病原菌监测中实施细菌耐药性监测具有较高的临床价值,有利于掌握抗菌药物的耐药情况、不同科室的致病菌分布,进而更好地指导后续治疗,保障患者抗菌药物用药安全,满足合理用药需求。Objective To explore the clinical value of bacterial resistance monitoring in clinical pathogen monitoring.Methods The microbial specimens of 117 patients with pathogenic bacteria infection detected in various departments of Gansu Wuwei Tumour Hospital from January to November 2023 were selected for microbiological examination and drug sensitivity test to analyze the monitoring effect of bacterial resistance.Results Among 117 strains of multidrug-resistant organism(MDRO),including 95 strains of escherichia coli,the highest proportion of drug resistance was Ciprofloxacin(77.89%);there were 12 strains of klebsiella pneumoniae,and the highest proportion of drug resistance was Sulfamethoxazole(25.00%);there were 10 strains of methicillin resistant Staphylococcus aureus(MRSA),and the highest proportion of drug resistance was Sulfamethoxazole(30.00%).From January to November 2023,the highest detection rates of MDRO were 29.73%and 28.57%in January and November.Among the 117 strains of MDRO,the detection rates of urine(27.35%)and sputum(17.09%)were the highest.In the distribution of departments,urology accounted for the highest proportion(13.68%),while in the age distribution,the detection rate of 71-80 years old was higher(37.50%).Conclusions The implementation of bacterial resistance monitoring in clinical pathogen monitoring has high clinical value,which is conducive to mastering the antibiotic resistance situation and the distribution of pathogenic bacteria in different departments,thus better guiding the follow-up treatment,guaranteeing the safety of patients'antibiotic medication,meeting the demand for rational medication.
