张西俭柴平加减方治疗湿滞互结型肥胖临床观察

摘　　要：目的探讨张西俭柴平加减方治疗湿滞互结型肥胖的临床疗效及对胰岛素抵抗的影响。方法选取医院2022年12月至2023年12月收治的湿滞互结型肥胖患者60例,随机分为对照组和观察组,各30例。两组患者均予基础饮食调理、运动指导,对照组患者加服平胃散,观察组患者加服张西俭柴平加减方。两组均治疗4周。结果观察组总有效率为96.67%,显著高于对照组的66.67%。治疗后,观察组患者的中医证候积分显著低于对照组(P<0.05);两组患者治疗后的体质量指数、甘油三酯(TG)、总胆固醇(TC)、空腹血糖(FBG)、低密度脂蛋白胆固醇(LDL-C)、稳态模型的胰岛素抵抗指数(HOMA-IR)均显著改善,且观察组患者的HOMA-IR显著低于对照组(P<0.05)。治疗前后两组患者的肝肾功能(丙氨酸氨基转移酶、天门冬氨酸氨基转氨酶、肌酐)均正常且不良反应发生率无显著差异(P>0.05)。结论张西俭柴平加减方治疗湿滞互结型肥胖,可有效调节患者糖脂代谢,改善胰岛素抵抗。Objective To investigate the clinical efficacy of Zhangxijian Modified Chaiping Formula in the treatment of obesity with dampness-stagnation interjunction syndrome and its effect on the insulin resistance.Methods Sixty obese patients with dampness-stagnation interjunction syndrome admitted to the hospital from December 2022 to December 2023 were selected and randomly divided into the control group and the observation group,with 30 cases in each group.Both groups were given basic dietary and exercise guidance,on this basis,the patients in the control group took Pingwei Powder,and the patients in the observation group took Zhangxijian Modified Chaiping Formula.All patients were treated for four weeks.Results The total effective rate in the observation group was 96.67%,which was significantly higher than 66.67%in the control group(P<0.05).After treatment,the traditional Chinese medicine(TCM)syndrome scores in the observation group were significantly lower than those in the control group(P<0.05);the body mass index(BMI),triglycerides(TG),total cholesterol(TC),fasting blood glucose(FBG),low-density lipoprotein cholesterol(LDL-C),and homeostasis model assessment of insulin resistance(HOMA-IR)in the two groups significantly improved,and HOMA-IR in the observation group was significantly lower than that in the control group(P<0.05).The liver and kidney functions(alanine aminotransferase,aspartate aminotransferase,creatinine)in the two groups were normal before and after treatment,and there was no significant difference in the incidence of adverse reactions(P>0.05).Conclusion Zhangxijian Modified Chaiping Formula can effectively regulate the glucose and lipid metabolism and improve insulin resistance in obese patients with dampness-stagnation interjunction syndrome.
