脉络宁口服液联合奥拉西坦治疗老年血管性痴呆临床观察

摘　　要：目的探讨脉络宁口服液联合奥拉西坦治疗老年血管性痴呆(VD)的临床疗效,以及对核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)炎性小体、神经元凋亡相关因子的影响。方法选取医院2020年8月至2023年8月收治的老年VD患者140例,按治疗方案的不同分为对照组(n=65)和观察组(n=75)。两组患者均予常规治疗及奥拉西坦胶囊口服,观察组患者加服脉络宁口服液。两组均连续治疗12周。结果观察组总有效率为94.67%,显著高于对照组的78.46%(P<0.05)。治疗后,两组患者的NLRP3、血清神经无凋亡相关因子(可溶性凋亡相关因子及其配体)、血流动力学指标(血浆黏度、全血高切黏度、全血低切黏度)、脂质过氧化物、丙二醛水平及日常生活能力评定量表、临床痴呆评定量表评分均显著降低(P<0.05),超氧化物歧化酶、谷胱甘肽过氧化物酶水平及蒙特利尔认知评估量表、简易精神状态检查量表评分均显著升高(P<0.05),且观察组均显著优于对照组(P<0.05)。对照组与观察组不良反应发生率相当(12.31%比12.00%,P>0.05),且均未发生严重不良反应。结论脉络宁口服液联合奥拉西坦治疗老年VD,可促进患者认知功能的恢复,减轻痴呆程度,改善脑部血流动力学,降低NLRP3、神经元相关凋亡因子和氧化应激指标水平,提高日常生活能力。Objective To investigate the clinical efficacy of Mailuoning Oral Solution combined with oxiracetam in the treatment of elderly vascular dementia(VD),as well as its effects on the nucleotide binding oligomerization domain-like receptor protein 3(NLRP3)inflammasome and neuronal apoptosis-related factors.Methods A total of 140 elderly VD patients admitted to the hospital from August 2020 to August 2023 were selected and divided into the control group(n=65)and the observation group(n=75)by the different treatment regimens.Both groups received routine treatment and oral Oxiracetam Capsules,while the observation group received additional treatment with Mailuoning Oral Solution.All patients were treated continuously for 12 weeks.Results The total effective rate in the observation group was 94.67%,which was significantly higher than78.46%in the control group(P<0.05).After treatment,the levels of NLRP3,serum neuronal apoptosis-related factors[soluble factor-related apoptosis(sFas)and sFas ligand(sFasL)],hemodynamic indicators(plasma viscosity,whole blood high-shear viscosity,whole blood low-shear viscosity),lipid hydroperoxide(LPO)and malondialdehyde(MDA)levels,the Activities of Daily Living(ADL)and Clinical Dementia Rating(CDR)scale scores significantly decreased in the two groups(P<0.05),and the superoxide dismutase(SOD),glutathione peroxidase(GSH-Px)levels,Montreal Cognitive Assessment(MoCA)scale,and Mini-Mental State Examination(MMSE)scale scores significantly increased(P<0.05);the above indicarors in the observation group were significantly better than those in the control group(P<0.05).The incidence of adverse reactions in the control group and observation group was similar(12.31%vs.12.00%,P>0.05),and both groups did not experience serious adverse reactions.Conclusion Mailuoning Oral Solution combined with oxiracetam can promote the recovery of cognitive function in elderly patients with VD,relieve dementia,improve cerebral hemodynamics,decrease NLRP3,neuronal apoptosis-related factors,and oxidative stress indicato
