固肠止泻丸治疗溃疡性结肠炎的Meta分析

摘　　要：目的系统评价固肠止泻丸对于溃疡性结肠炎的临床疗效、不良反应、肿瘤坏死因子-α(TNF-α)及白细胞介素-8(IL-8水平的改善作用。方法检索建库至2023年12月31日的所有关于固肠止泻丸治疗溃疡性结肠炎的临床随机对照试验(RCT),中文数据库包括中国学术期刊、万方、中国生物医药等,英文数据库包括Pub Med、Emerald、Web of Science等,对文献进行筛选,提取数据进行统计学分析。结果共有10个研究被纳入,总计849例患者。Meta分析结果:治疗组的临床疗效更高[OR=4.24,95%CI(2.59,6.96),P<0.00001]、TNF-α水平更低[MD=-2.01,95%CI(-4.16,0.13),P=0.07]、IL-8水平也更低[MD=-45.86,95%CI(-84.51,-7.21),P=0.02],两组不良反应未见明显差异[OR=0.80,95%CI(0.44,1.44),P=0.45]。结论固肠止泻丸治疗溃疡性结肠炎临床疗效较好,降低炎症因子效果显著,且具备较高的安全性,但依旧需要足够样本量的高质量临床随机对照试验对此结论进行论证。Objective To systematically evaluate the clinical efficacy,adverse drug reaction,and improvement of tumor necrosis factor-α(TNF-α)and interleukin-8(IL-8)levels of Guchang Zhixie Pills in the treatment of ulcerative colitis.Methods All clinical randomized controlled trials(RCTs)on the treatment of ulcerative colitis with Guchang Zhixie Pills from the establishment of the database to December 31,2023 were searched.The Chinese database included Chinese academic journals,Wanfang,and Chinese biomedicine.The English database included PubMed,Emerald,Web of Science,etc.The literature was screened,the data were extracted,and statistical analysis was performed.Results A total of 10 studies involving 849 patients were included.The results of Meta-analysis showed that the clinical efficacy of the treatment group was higher[OR=4.24,95%CI(2.59,6.96),P<0.00001],the level of TNF-αwas lower[MD=-2.01,95%CI(-4.16,0.13),P=0.07],and the level of IL-8 was lower[MD=-45.86,95%CI(-84.51,-7.21),P=0.02].There was no significant difference in adverse drug reaction between the two groups[OR=0.80,95%CI(0.44,1.44),P=0.45].Conclusion The clinical efficacy of Guchang Zhixie Pills in the treatment of ulcerative colitis is good,the effect of reducing inflammatory factors is significant,and it has high safety.However,high-quality clinical RCT with sufficient sample size are still needed to demonstrate this conclusion.
