摘　　要：目的探究和分析PDCA循环管理法对基层医疗机构住院患者抗菌药物合理使用的管控效果。方法回顾性收集某基层医疗机构2022年5月~2023年4月住院患者抗菌药物使用数据作为基线,2023年5月~2024年4月运用PDCA质量管理工具分析抗菌药物使用不合理和监测指标未达标的原因。实施持续提高抗菌药物临床合理使用水平的解决对策,并逐步形成标准化的质量管理体系。结果应用PDCA循环管理法干预后,该医疗机构住院患者抗菌药物使用率于2024年4月下降至56.23%,住院患者抗菌药物使用强度下降至32.46DDDs,Ⅰ类切口手术预防性使用抗菌药物比例下降至20.00%,均达到PDCA管理目标。结论科学应用PDCA质量管理工具可以有效改善基层医疗机构住院患者抗菌药物的不合理使用情况,PDCA循环管理法是一种行之有效、可持续、可复制的抗菌药物管理方法。Objective To investigate and analyze the effectiveness of the PDCA cycle management method in controlling the rational use of antimicrobial drugs among inpatients in primary healthcare institutions.Methods The antimicrobial drugs use data of inpatients at a certain primary healthcare institutions from May 2022 to April 2023 were retrospectively collected as the baseline.From May 2023 to April 2024,the PDCA quality management tool was used to analyze the reason for irrational use of antimicrobial drugs among inpatients and unmet indicators.Solutions were explored and implemented to continuously enhance the clinical rational use of antimicrobial drugs,and a standardized quality management system was gradually established.Results After the intervention of PDCA cycle management method,the utilization rate of antibacterial drug in inpatients decreased to 56.23%,the intensity of antimicrobial drugs for inpatients decreased to 32.46 DDDs,and the use of antimicrobial drugs for typeⅠincision surgery prophylaxis decreased to 20.00%,all of which reached the management target of PDCA.Conclusion The scientific application of the PDCA quality management tool could effectively improve the irrational usage of antimicrobial drugs among inpatients in primary healthcare institutions.The PDCA cycle management method is an effective,sustainable,and replicable method for managing antimicrobial drugs.
