摘 要:目的探讨3D打印技术在凶险型前置胎盘中的临床应用。方法选择2022年1月—2023年6月在江西省人民医院就诊的凶险型前置胎盘患者60例,随机分成观察组(30例)和对照组(30例)。两组均行超声联合磁共振成像(MRI),观察组再行MRI三维重建及打印。比较两组产前诊断准确性、手术相关指标、新生儿出生及预后情况。结果观察组的失血量、输血率及手术时间分别为(765.65±249.50)ml、40.00%(12例)、(63.59±23.56)min;对照组的失血量、输血率及手术时间分别为(950.36±305.65)ml、86.67%(26例)、(83.75±49.35)min。两组失血量、输血率及手术时间比较,差异均有统计学意义(均P<0.05)。观察组灵敏度和阴性预测值分别为96.67%和90.00%,对照组灵敏度和阴性预测值分别为80.00%和56.67%,两组比较差异均有统计学意义(均P<0.05)。观察组和对照组入重症监护室新生儿例数分别为14例(46.67%)和23例(76.67%),差异有统计学意义(χ^(2)=5.71,P<0.05)。结论3D打印技术灵敏度和阴性预测值较高,减少了失血量,降低了输血率、缩短了手术时间,新生儿转诊率低,对凶险型前置胎盘的诊治有积极作用。Objective To explore the clinical application of 3D printing technology in pernicious placenta previa.Methods A total of 60 pernicious placenta previa patients admitted to Jiangxi Provincial People's Hospital from January 2022 to June 2023 were selected and randomly divided into observation group(30 cases)and control group(30 cases).Ultrasound combined with magnetic resonance imaging(MRI)was performed in the two groups,MRI 3D reconstruction and printing were performed in observation group.The accuracies of prenatal diagnosis,operation-related indexes,neonatal birth and prognosis were compared between the two groups.Results The volume of blood loss,the rate of blood transfusion,and operation time in observation group were(765.65±249.50)ml,40.00%(12 cases),and(63.59±23.56)minutes,respectively;the volume of blood loss,the rate of blood transfusion,and operation time in control group were(950.36±305.65)ml,86.67%(26 cases),and(83.75±49.35)minutes,respectively.There were statistically significant differences in the volume of blood loss,the rate of blood transfusion,and operation time between the two groups(P<0.05).The sensitivity and negative predictive value in observation group were 96.67%and 90.00%,respectively,the sensitivity and negative predictive value in control group were 80.00%and 56.67%,respectively,there were statistically significant differences between the two groups(P<0.05).The numbers of neonates admitted to intensive care unit in observation group and control group were 14 cases(46.67%)and 23 cases(76.67%),respectively,there was statistically significant difference(χ^(2)=5.71,P<0.05).Conclusion 3D printing technology has high sensitivity and negative predictive value,reduces blood loss and blood transfusion,shortens operation time with low neonatal referral rate,which plays a positive role in diagnosis and treatment of pernicious placenta previa.
