摘 要:目的探讨妊娠期高血压疾病患者脑钠肽(BNP)、尿微量白蛋白(mALB)及凝血功能变化与病情严重程度的相关性,为妊娠期高血压疾病的诊治提供参考。方法选取2020年4月—2023年3月在舟山市妇女儿童医院就诊的100例妊娠期高血压疾病患者为研究组,根据患者病情严重程度再分为妊娠期高血压组(72例)与子痫前期组(28例),选取同期产检正常妊娠晚期孕妇100例为对照组,检测血清BNP、mALB、凝血功能指标凝血酶原时间(PT)、部分凝血活酶时间(APTT)、凝血酶时间(TT)及纤维蛋白原(FIB)水平。结果对照组、妊娠期高血压组及子痫前期组孕妇血清BNP为(152.37±34.85)pg/ml、(276.12±42.97)pg/ml、(331.08±56.96)pg/ml,mALB为(8.86±4.09)mg/L、(34.95±7.85)mg/L、(56.13±9.61)mg/L,凝血指标APTT为(32.07±2.02)s、(30.33±1.38)s、(27.56±1.74)s,TT为(14.26±0.97)s、(13.12±0.94)s、(12.32±0.90)s,FIB为(4.23±0.47)s、(5.34±0.52)s、(6.10±0.43)s,差异均有统计学意义(均P<0.05),各组PT水平分别为(11.34±0.78)s、(11.28±0.75)s、(11.26±0.74)s,差异无统计学意义(P>0.05);研究组孕妇血清BNP为(285.69±50.77)pg/ml,mALB为(48.61±3.41)mg/L,凝血指标APTT为(29.03±1.56)s,TT为(12.84±0.93)s,FIB为(5.87±0.48)s,对照组与研究组比较,差异均有统计学意义(均P<0.05),两组PT比较,差异无统计学意义(P>0.05);妊娠期高血压组与子痫前期组孕妇血清BNP、mALB、APTT、FIB比较,差异均有统计学意义(均P<0.05),两组PT比较,差异无统计学意义(P>0.05)。Spearman相关分析显示,随着妊娠期高血压疾病患者病情加重,BNP、mALB及FIB随之升高,差异有统计学意义(P<0.05),APTT、TT随之下降,差异有统计学意义(P<0.05)。结论妊娠期高血压疾病患者BNP、mALB及凝血功能与病情严重程度密切相关,通过检测BNP、mALB及凝血功能指标有助于判断病情严重程度,为预后的评估及病情的诊治提供参考。Objective To investigate the correlation between changes in brain natriuretic peptide(BNP),microalbuminuria(mALB),and coagulation function and the severity of gestational hypertension,provide reference for diagnosis and treatment of gestational hypertension.Methods A total of 100 patients with gestational hypertension who visited Zhoushan Women's and Children's Hospital from April 2020 to March 2023 were selected as the study group.According to the severity of the condition,they were further divided into gestational hypertension group(72 cases)and preeclampsia group(28 cases).A control group of 100 pregnant women with normal late pregnancy who underwent prenatal examinations during the same period were selected.Serum BNP,mALB,coagulation function indicators prothrombin time(PT),partial thromboplastin time(APTT),thrombin time(TT),and fibrinogen(FIB)levels were measured.Results The serum BNP levels of pregnant women in the control group,gestational hypertension group,and preeclampsia group were(152.37±34.85)pg/ml,(276.12±42.97)pg/ml,(331.08±56.96)pg/ml,mALB is(8.86±4.09)mg/L,(34.95±7.85)mg/L,(56.13±9.61)mg/L,coagulation index APTT is(32.07±2.02)s,(30.33±1.38)s,(27.56±1.74)s,TT is(14.26±0.97)s,(13.12±0.94)s,(12.32±0.90)s,FIB is(4.23±0.47)s,(5.34±0.52)s,(6.10±0.43)s,with statistically significant differences(all P<0.05),the PT levels of each group were(11.34±0.78)s,(11.28±0.75 seconds,(11.26±0.74)s,with no statistically significant difference(P>0.05);The serum BNP level of pregnant women in the research group was(285.69±50.77)pg/ml,mALB is(48.61±3.41)mg/L,the coagulation index APTT is(29.03±1.56)s,TT is(12.84±0.93)s,FIB is(5.87±0.48)s,the difference between the control group and the study group was statistically significant(all P<0.05),there was no statistically significant difference in PT between the two groups(P>0.05);There were statistically significant differences in serum BNP,mALB,APTT,and FIB levels between pregnant women with gestational hypertension and those with preeclampsia(all P<0.
