快速康复外科理念下的中医护理在恶性骨肿瘤患者中的应用效果

摘　　要：目的探讨快速康复外科(ERAS)理念下的中医护理在行放射治疗和化学治疗(简称“放化疗”)的恶性骨肿瘤患者中的应用情况。方法以2022年6月至2023年6月于华中科技大学同济医学院附属同济医院肿瘤科行放化疗的108例恶性骨肿瘤患者为研究对象,根据随机数字表法将其分为对照组(常规护理)和观察组(常规护理^(+)基于ERAS理念的中医护理),各54例。比较2组患者的免疫相关指标、炎症相关指标、疼痛程度评分和生活质量评分之间的差异。结果干预后,2组的外周血T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+))、免疫球蛋白G、免疫球蛋白M和免疫球蛋白A水平均较干预前升高(P均<0.05),且观察组均高于对照组(P均<0.05)。干预后,2组的白细胞计数和C反应蛋白水平均低于干预前(P均<0.05),且观察组均低于对照组(P均<0.05)。干预后,2组的简化McGill疼痛问卷量表各维度评分均低于干预前,癌症治疗功能评价系统-普适性量表评分均高于干预前(P均<0.05),且观察组均较对照组更优(P均<0.05)。结论ERAS理念下的中医护理可改善行放化疗的恶性骨肿瘤患者的免疫功能,减轻其炎症反应和疼痛感,提高其生活质量。Objective To investigate the application of traditional Chinese Medicine nursing under the concept of enhanced recovery after surgery(ERAS)in patients with malignant bone tumors undergoing radiotherapy and chemotherapy(referred to as"radiochemotherapy").Methods A total of 108 patients with malignant bone tumors who underwent radiochemotherapy at Tongji Hospital,Tongji Medical College,Huazhong University of Science and Technology from June 2022 to June 2023 were enrolled.They were divided into a control group(conventional nursing)and an observation group(conventional nursing^(+)traditional Chinese medicine nursing under the ERAS concept)using a random number table,each with 54 patients.The differences in immune-related indicators,inflammation-related indicators,pain intensity scores,and quality of life scores of the two groups were compared.Results After the intervention,the peripheral blood T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+)),immunoglobulin G,immunoglobulin M,and immunoglobulin A levels increased in both groups compared with those before the intervention(all P<0.05),and the observation group had higher levels than the control group(all P<0.05).After the intervention,the white blood cell count and C-reactive protein level decreased in both groups compared with those before the intervention(all P<0.05),and the observation group had lower levels than the control group(both P<0.05).After the intervention,the scores of each dimension of the Short-Form McGill Pain Questionnaire in both groups were lower than those before the intervention,and the scores of the Functional Assessment of Cancer Therapy-General were higher than those before the intervention(all P<0.05).The observation group had better scores than the control group(all P<0.05).Conclusion Traditional Chinese medicine nursing under the ERAS concept can improve the immune function,reduce the inflammatory reaction and pain,and increase the quality of life in patients with malignant bone tumors undergoing radiochemotherapy.
