中青年腹膜透析患者压力感知的质性研究

摘　　要：目的:探讨中青年终末期肾脏病患者腹膜透析期间的压力感知情况,为医护人员采取针对性干预措施提供依据。方法:采用现象学研究方法对19例中青年腹膜透析患者进行半结构式访谈,运用Colaizzi 7步分析法和Nvivo 12.0对访谈资料进行整理分析。结果:共提炼出身体负担加重、情绪变化多样、工作变动巨大、经济负担加重、生活质量下降、操作繁重严格、社会交往改变、应对方式多样及社会支持需求增加9个主题。结论:中青年腹膜透析患者面临多种压力,家人朋友、医护人员和社会可以从身体、心理、工作、经济、社会交往等方面给予患者帮助,引导患者采取积极的压力应对方式,以缓解压力。Objective:To explore the perceived stress among young and middle-aged patients with end-stage renal disease undergoing peritoneal dialysis,providing a basis for targeted interventions by healthcare providers.Methods:A phenomenological approach was used to conduct semi-structured interviews with 19 young and middle-aged patients undergoing peritoneal dialysis.Colaizzi′s 7-step method and Nvivo 12.0 were applied to organize and analyze the interview data.Results:Nine themes were identified,including increased physical burden,diverse emotional changes,significant job changes,heightened economic burden,decreased quality of life,demanding and strict dialysis procedures,altered social interactions,diverse coping strategies,and increased need for social support.Conclusion:Young and middle-aged patients undergoing peritoneal dialysis face various stresses.Family,friends,healthcare providers,and society can offer support in areas such as physical,psychological,work,economic,and social aspects,and guide patients to adopt positive coping strategies to alleviate stress.
