甘油三酯-葡萄糖指数与慢性心力衰竭左心室整体纵向应变的关系研究

摘　　要：目的探讨甘油三酯-葡萄糖(TyG)指数与慢性心力衰竭(下称心衰)左心室整体纵向应变(LVGLS)的关系。方法回顾性选取2020年1月至2023年5月金华广福肿瘤医院收治的慢性心衰患者190例,根据TyG指数三分位数分成3组,比较不同TyG指数患者临床资料,采用Pearson相关和多元线性回归分析TyG指数与LVGLS的关系。结果低TyG指数(<8.40)组60例,中TyG指数(8.40~8.90)组61例,高TyG指数(>8.90)组69例。不同TyG指数患者在合并糖尿病比例、合并高脂血症比例、纽约心脏病协会心功能分级、使用降糖药物比例、总胆固醇水平、低密度脂蛋白胆固醇水平、高密度脂蛋白胆固醇水平以及LVGLS等方面比较,差异均有统计学意义(均P<0.05)。Pearson相关显示,TyG指数与LVGLS呈负相关(r=-0.360,P=0.010)。采用多元线性回归构建3个模型(调整混杂因素)发现,TyG指数均与LVGLS独立相关(均P<0.01)。结论慢性心衰患者TyG指数与LVGLS独立相关,监测TyG指数有助于预判左心室功能障碍风险。Objective To explore the relationship between triglyceride-glucose(TyG)index and left ventricular global longitudinal strain(LVGLS)in chronic heart failure.Methods A retrospective study was conducted on 190 patients with chronic heart failure admitted to Jinhua Guangfu Cancer Hospital from January 2020 to May 2023.According to the third quartile of TyG index,patients were divided into three groups,and the clinical data in different groups were compared.Pearson correlation and multiple linear regression were used to analyze the relationship between TyG index and LVGLS.Results There were 60 cases in the low TyG index(<8.40)group,61 cases in the medium TyG index(8.40-8.90)group,and 69 cases in the high TyG index(>8.90)group.Patients with different TyG indexes showed significantly differences in the proportions of diabetes,hyperlipidemia,heart function grade of New York Heart Association,usage of hypoglycemic drugs,levels of total cholesterol,low-density lipoprotein cholesterol and high-density lipoprotein cholesterol,and LVGLS(all P<0.05).Pearson correlation analysis showed a negative correlation between TyG index and LVGLS(r=-0.360,P=0.010).By using multiple linear regression to construct three models(adjusting for confounding factors),it was found that TyG index was an independent influencing factor of LVGLS(all P<0.01).Conclusion TyG index is independently correlated with LVGLS in patients with chronic heart failure,and monitoring the TyG index can help predict the risk of left ventricular dysfunction.
