不同剂量舒芬太尼复合罗哌卡因蛛网膜下腔麻醉对中老年肛肠手术术后镇痛的影响

摘　　要：目的:本研究探索不同剂量舒芬太尼复合罗哌卡因在蛛网膜下腔麻醉对中老年人肛肠手术术后镇痛的影响。方法:选取2023年2月—2024年2月内蒙古自治区中医医院手术室行肛肠手术的患者90例分析,将病例随机分为3组,每组30例。患者均行经L3~4的蛛网膜下腔麻醉。A组:罗哌卡因0.8 mg,用脑脊液稀释到1.5 ml;B组:舒芬太尼3 ug复合罗哌卡因0.8 mg,用脑脊液稀释到1.5 ml;C组:舒芬太尼5 ug复合罗哌卡因0.8 mg,用脑脊液稀释到1.5 ml。主要观察指标为患者术后6 h、12 h、24 h的VAS评分;次要观察指标为术后不良反应发生率,并及时给与中医针刺或按摩穴位疗法处理。结果:B组、C组与A组术后6 h、12 h、24 h比较,P<0.05,有统计学意义。而B组与、C组比较P>0.05,无统计学意义。结论:舒芬太尼3.0 ug复合罗哌卡因用于肛肠术后镇痛可以达到满意的效果且并发症较少。Objective:This study investigated the effect of subarachnoid anesthesia with different doses of sufentanil combined with ropiva caine on postoperative analgesia after anorectal surgery.Method:A total of 90 patients who underwent anorectal surgery in the operating room of the Hospital of Traditional Chinese Medicine of Inner Mongolia Autonomous Region from February 2023 to February 2024 were selected for analysis and randomly divided into 3 groups with 30 patients in each group.All patients underwent subarachnoid anesthesia through L3-4.Group A:Ropivacaine 0.8mg,diluted with cerebrospinal fluid to 1.5ml;Group B:Sufentanil 3ug and ropivacaine 0.8mg,diluted with cere brospinal fluid to 1.5ml;Group C:Sufentanil 5ug and ropivacaine 0.8mg diluted with cerebrospinal fluid to 1.5ml.The main outcome mea sures were VAS scores at 6h,12h and 24h after surgery.The secondary outcome measures were postoperative adverse reactions and timely treatment with traditional Chinese medicine.Result:The comparison between group B,C and group A at 6h,12h and 24h after operation was statistically significant(P<0.05).The comparison between group B and group C showed no statistical significance(P>0.05).Conclusion:Sufentanil 3.0ug combined with ropivacaine for postoperative anorectal analgesia can achieve satisfactory results with fewer complications.
