中医治疗功能性腹泻的研究概况

摘　　要：功能性腹泻(functional diarrhea,FDr)是一类具有消化道症状而无法用器质性病变或生化指标异常来解释的疾病。目前西医认为胃肠动力障碍、内脏高敏感性、胃肠电生理紊乱、肠道微生态紊乱、胃肠激素分泌异常、脑-肠相互作用,脑-肠肽变化,环境因素、心理因素等可能是引起功能性腹泻的原因。功能性腹泻的发病机制仍不明确,药物治疗可缓解症状,但停药后复发的几率较高,呈反复性,效果不甚理想。中医药及中医特色疗法等防治FDr具有良好的临床实践基础及应用,中医在治疗上具有多途径、多靶点的作用特征,具有高效、创伤性小的优点。中医针刺、穴位贴敷、灸法,中医特色疗法对临床治疗具有重要的辅助治疗作用,可有效刺激局部腧穴部位深层及近周的组织机能,使局部血管扩张,促进血液循环,影响胃肠道特定受体表达,改善胃肠道功能,提高患者满意度。本文主要对近10年来中医药治疗FDr的文献进行系统梳理总结和分析,以期为FDr的临床治疗提供思路与参考。Functional diarrhea(FDr)is a kind of disease with digestive tract symptoms,which can not be explained by organic lesions or abnormal biochemical indexes At present,Western medicine believes that gastrointestinal motility,visceral hyperactivity,gastrointestinal electrophysiological disorder,intestinal microecological disorder,gastrointestinal abnormality,brain-intestine interaction,brain-intestine peptide changes,psychological factors and so on may be the causes of functional diarrhea Drug treatment can relieve symptoms,but the recurrence rate is higher after drug withdrawal,and the repetitive effect is not very good Traditional Chinese medicine and traditional Chinese medicine characteristic therapy have good clinical practice foundation for preventing and treating FDr.The application of traditional Chinese medicine in treatment has the characteristics of multi-channel and multi-target,and has the advantages of high efficiency and less trauma Traditional Chinese medicine acupoint application and other special therapies have an important adjuvant therapeutic effect on clinical treatment,which can effectively expand local blood vessels,promote blood circulation,affect the expression of specific receptors in gastrointestinal tract,improve gastrointestinal tract function and improve patient satisfaction However,the pathogenesis of functional diarrhea is still unclear,so the pathogenesis of FDR can be further studied The author reviewed the etiology,pathogenesis and characteristic treatment of FDR from the aspect of TCM by consulting the relevant literatures in recent 10 years in order to provide reference for further clinical research.
