摘 要:目的探讨高剂量重组人生长激素对特发性矮小症患儿生长发育的影响。方法选取2020年3月—2022年3月青岛市即墨区中医医院收治的84例特发性矮小症患儿为研究对象,按随机数字表法将其分为对照组和观察组,每组42例。对照组采用低剂量重组人生长激素治疗,观察组采用高剂量重组人生长激素治疗。比较两组的生长因子及相关结合蛋白、生长发育指标、甲状腺功能、不良反应发生情况。结果治疗前,两组生长因子及相关结合蛋白、生长发育指标、甲状腺功能比较,组间差异无统计学意义(P>0.05);治疗后,观察组胰岛素样生长因子-1、胰岛素样生长因子结合蛋白-3水平为(329.25±65.64)μg/L、(5.58±1.13)μg/L,均高于对照组的(273.43±54.27)μg/L、(4.36±2.15)μg/L,组间差异有统计学意义(P<0.05);观察组身高高于对照组,体重重于对照组,生长速度快于对照组,身高标准差分值优于对照组,组间差异有统计学意义(P<0.05);两组血清游离三碘甲状腺原氨酸、血清游离甲状腺素、促甲状腺激素水平、不良反应发生率比较,组间差异无统计学意义(P>0.05)。结论对特发性矮小症患儿采用不同剂量重组人生长激素治疗,高剂量重组人生长激素效果更为显著,可有助于改善血清胰岛素样生长因子水平,加速患儿生长发育,且对甲状腺功能无影响,较为安全。Objective To explore the impact of high-dose recombinant human growth hormone on the growth and development of children with idiopathic short stature.Methods Eighty-four children with idiopathic short stature who were admitted to Jimo District Hospital of Traditional Chinese Medicine in Qingdao from March 2020 to March 2022 were selected as the research objects,they were divided into a control group and an observation group according to the random number table method,with 42 cases in each group.The control group was treated with low-dose recombinant human growth hormone,while the observation group was treated with high-dose recombinant human growth hormone.The growth factors and related binding proteins,growth and development indicators,thyroid function,and the occurrence of adverse reactions were compared between the two groups.Results Before treatment,the growth factors and related binding proteins,growth and development indicators,and thyroid function were compared between the two groups,and the differences between the groups were no statistically significant(P>0.05);after treatment,the levels of insulin-like growth factor-1 and insulin-like growth factor binding protein-3 in the observation group were(329.25±65.64)μg/L and(5.58±1.13)μg/L,respectively,which were higher than(273.43±54.27)μg/L and(4.36±2.15)μg/L in the control group,and the differences between the groups were statistically significant(P<0.05);the body height in the observation group was higher than that in the control group,the body weight was heavier than that of the control group,the growth rate was faster than that of the control group,and the height standard deviation score was better than that of the control group,and the differences between the groups were statistically significant(P<0.05);the levels of serum free triiodothyronine,serum free thyroxine,thyroid-stimulating hormone and the incidence of adverse reactions were compared between the two groups,and the differences between the groups were no statistically significant(P>0.05
