动力髋螺钉与髋部联合加压交锁髓内钉系统固定术治疗老年股骨粗隆间骨折患者的临床效果

摘　　要：目的比较老年股骨粗隆间骨折(FIF)患者采用髋部联合加压交锁髓内钉系统(InterTan)与动力髋螺钉(DHS)固定术治疗的效果。方法选取2023年1月—2023年12月菏泽市第六人民医院收治的92例老年FIF患者为研究对象,按随机数字表法将其分为对照组和研究组,每组46例。对照组采用DHS固定术,研究组采用InterTan固定术。比较两组患者的围手术期情况、髋关节功能及术后并发症发生情况。结果研究组术中操作时间、骨折愈合时间均短于对照组,术中出血量少于对照组,组间差异有统计学意义(P<0.05);两组离床行走时间比较,差异无统计学意义(P>0.05)。术后,研究组Harris髋关节功能评分量表评分高于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率为2.17%,低于对照组的17.39%,差异有统计学意义(P<0.05)。结论相较于DHS,InterTan固定术治疗老年FIF具有手术时间短、出血量少等优势,且可以加快骨折愈合速度,促进髋关节功能恢复,降低并发症发生风险。Objective Comparison of the effect of TriGen InterTan hip fracture nailing system(InterTan)and dynamic hip screw(DHS)fixation in elderly patients with femoral intertrochanteric fracture(FIF).Methods A total of 92 elderly patients with FIF treated in the Heze Sixth People's Hospital from January 2023 to December 2023 were selected as the research objects,and were divided into a control group and a research group by random number table method,with 46 cases in each group.The control group underwent DHS fixation,while the research group underwent InterTan fixation.The perioperative status,hip function and postoperative complications were compared between the two groups.Results The intraoperative operation time and fracture healing time of the research group were shorter than those of the control group,and the intraoperative blood loss was less than that of the control group,the differences were statistically significant(P<0.05).There was no significant difference in walking time between the two groups(P>0.05).After operation,the Harris Hip Scale score of the research group was higher than that of the control group,and the difference was statistically significant(P<0.05).The incidence of postoperative complications in the research group was 2.17%,which was lower than 17.39%in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with DHS,InterTan internal fixation in the treatment of elderly FIF has the advantages of shorter operation time and less bleeding,and can accelerate the healing speed of fracture,promote the recovery of hip function,and reduce the risk of complications.
