人工股骨头置换术与人工全髋关节置换术对老年股骨颈骨折患者疼痛程度、髋关节功能的影响比较

摘　　要：目的比较人工股骨头置换术与人工全髋关节置换术对老年股骨颈骨折患者疼痛程度、髋关节功能的影响。方法选取2022年3月—2023年4月寿光市人民医院收治的86例老年股骨颈骨折患者为研究对象,根据手术方式的不同将其分为A组和B组,每组43例。A组采用人工股骨头置换术治疗,B组采用人工全髋关节置换术治疗。比较两组患者的围术期指标、术后疼痛程度、髋关节功能、并发症发生情况。结果术前,两组髋关节功能比较,差异无统计学意义(P>0.05)。B组手术时间长于A组,住院费用高于A组,组间差异有统计学意义(P<0.05);术后6 h,B组疼痛视觉模拟评分为(3.15±0.41)分,低于A组的(4.07±1.17)分,差异有统计学意义(P<0.05);术后6个月,B组Harris髋关节功能量表评分高于A组,差异有统计学意义(P<0.05);两组下地活动时间、住院天数、并发症发生率比较,组间差异无统计学意义(P>0.05)。结论两种术式安全性相当,但人工全髋关节置换术在减轻术后疼痛、改善髋关节功能方面更具有优势,人工股骨头置换术手术时间更短、费用更低。Objective Comparison of the effects of hemiarthroplasty of the femoral head and total hip arthroplasty on the pain level and hip joint function of elderly patients with femoral neck fractures.Methods Eighty-six elderly patients with femoral neck fractures who were admitted to Shouguang People's Hospital from March 2022 to April 2023 were selected as the research objects.According to different surgical methods,they were divided into a group A and a group B,with 43 cases in each group.Group A was treated with hemiarthroplasty of the femoral head,while group B was treated with total hip arthroplasty.The perioperative indicators,degree of postoperative pain,hip joint function,and occurrence of complications were compared between the two groups.Results Before the operation,the hip joint functions of the two groups were compared,and the difference was no statistically significant(P>0.05).The operation time in group B was longer than that in group A,and the hospitalization expenses in group B were higher than those in group A,and the differences between the groups were statistically significant(P<0.05);six hours after the operation,the pain Visual Analogue Scale score in group B was(3.15±0.41)points,which was lower than(4.07±1.17)points in group A,and the difference was statistically significant(P<0.05);six months after the operation,the Harris Hip Scale score in group B was higher than that in group A,and the difference was statistically significant(P<0.05);the time of getting out of bed for activities,the length of hospital stay,and the incidence of complications were compared between the two groups,and the differences between the groups were not statistically significant(P>0.05).Conclusion The safety of the two surgical procedures is comparable,however,total hip arthroplasty has more advantages in relieving postoperative pain and improving hip joint function,hemiarthroplasty of the femoral head has a shorter operation time and lower costs.
