肾上腺嗜铬细胞瘤患者黑色素A表达与肿瘤良恶性严重程度的相关性

摘　　要：目的分析肾上腺嗜铬细胞瘤患者黑色素A表达与肿瘤良恶性严重程度的相关性。方法选取2021年7月至2023年7月在本院收治的82例肾上腺嗜铬细胞瘤患者的临床资料,根据病理结果将患者分为良性组(40例)和恶性组(42例)。检测两组患者的白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、肿瘤坏死因子(TNF-α)、黑色素A表达水平。结果两组肿瘤侧别、分化程度、浸润程度、淋巴结转移发生率比较,差异均有统计学意义(均P<0.05);恶性组的IL-1β、IL-6、TNF-α水平高于良性组,差异均有统计学意义(均P<0.05)。恶性组的黑色素A水平低于良性组,差异有统计学意义[(48.77±10.33)μmol/L vs.(58.46±14.65)μmol/L,P<0.05]。与轻度PCC患者比较,重度PCC患者的黑色素A表达水平显著下降;在PCC患者中,与Ⅰ期相比,Ⅱ、Ⅲ和Ⅳ期的黑色素A表达水平均降低,其中Ⅳ期患者降低最为明显,差异均有统计学意义(均P<0.05)。恶性组的黑色素A阳性表达率高于良性组,差异有统计学意义(76.19%vs.40.00%,P<0.05);进一步经Spearman相关性分析结果显示,两组患者的肿瘤良恶性严重程度与黑色素A表达水平呈负相关,差异有统计学意义(P<0.05)。结论恶性肾上腺嗜铬细胞瘤患者的黑色素A表达呈降低趋势,其表达水平的异常升高或降低为肾上腺嗜铬细胞瘤患者的风险因子,并与患者的肿瘤良恶性严重程度具有一定关联性。Objective To analyze the correlation between the expression of melanoma A and benign and malignant toumor severity in adrenal pheochromocytoma patients.Methods The clinical data of 82 patients with adrenal pheochromocytoma,who were admitted to our hospital from July 2021 to July 2023,were selected.Based on the pathological results,the patients were categorized into two groups:benign(40 cases)and malignant(42 cases).The expression levels of interleukin-1β(IL-1β),interleukin-6(IL-6),tumor necrosis factor(TNF-α),and melanoma A were also tested in both groups.Results There were statistically significant differences in tumor side,differentiation degree,invasion degree and incidence of lymph node metastasis between the two groups(all P<0.05).The levels of IL-1β,IL-6 and TNF-αin malignant group were higher than those in benign group,and the differences were statistically significant(all P<0.05).The level of melanoma A in malignant group was lower than that in benign group,and the difference was statistically significant[(48.77±10.33)μmol/L vs.(58.46±14.65)μmol/L,P<0.05].Compared with patients with mild PCC,the expression level of melanoma A in patients with severe PCC was significantly decreased.In PCC patients,compared with stageⅠ,the expression level of melanoma A in stageⅡ,ⅢandⅣwas decreased,and the decrease was most obvious in stageⅣpatients,the difference was statistically significant(all P<0.05).The positive expression rate of melanoma A in malignant group was higher than that in benign group,and the difference was statistically significant(76.19%vs.40.00%,P<0.05).Further Spearman correlation analysis showed that the benign and malignant toumor severity of the two groups were negatively correlated with the expression of melanin A level,and the difference was statistically significant(P<0.05).Conclusions The expression of melanoma A in patients with malignant adrenal pheochromocytoma tends to decrease,and its abnormal increase or decrease is a risk factor for adrenal pheochromocytoma patients,and
