摘　　要：目的探讨经皮肾镜1470 nm激光去顶术与肾囊肿穿刺硬化术治疗单纯性肾囊肿的疗效比较。方法选取2018年4月至2020年4月在本院收治的76例单纯肾囊肿患者的临床资料,按照不同治疗方式将患者分为试验组和对照组,每组各38例。试验组采用经皮肾镜1470 nm激光去顶术,对照组采用超声引导下肾囊肿穿刺硬化术。比较两组患者的术中出血量、术后住院时间和手术操作时间等手术指标。分别测定两组的尿肾损伤因子-1(KIM-1)、血清胱抑素C(Cys C)、血肌酐(Scr)和尿素氮水平。术后随访6个月,比较两组患者出现感染、出血、发热和复发等并发症的发生率。结果与对照组相比,试验组患者的术中出血量更少、住院时间更短,且手术操作时间更短,差异均有统计学意义(均P<0.05)。两组患者术后的血Scr、血尿素氮及血清Cys C均较治疗前下降,而尿KIM-1较治疗前上升(均P<0.05)。试验组术后的血Scr、血尿素氮和血清Cys C均低于对照组,而尿KIM-1高于对照组,差异均有统计学意义(均P<0.05)。试验组患者的并发症发生率低于对照组,差异有统计学意义(t=5.330,P=0.021)。结论相较于肾囊肿穿刺硬化术,经皮肾镜1470 nm激光去顶术的术中出血量更少、住院时间更短、手术时间更短,术后复发率更低。Objective To explore the comparison of efficacy between percutaneous nephroscopy 1470 nm laser unroofing and renal cyst puncture sclerotherapy in the treatment of simple renal cyst.Methods The clinical data of 76 patients with simple renal cysts who were admitted to our hospital from April 2018 to April 2020 were selected and divided into an experimental group and a control group according to different treatment methods,with 38 cases in each group.The experimental group had percutaneous nephroscopic 1470 nm laser unroofing,whereas the control group received ultrasound-guided renal cyst puncture and sclerotherapy.The surgical indicators such as intraoperative blood loss,postoperative hospital stay,and surgical procedure duration were compared between the two groups.The levels of urine kidney injury factor-1(KIM-1),serum cystatin C(Cys C),serum creatinine(Scr),and urea nitrogen were determined in both groups.The incidence of complications such as in-fection,hemorrhage,fever,and recurrence was compared between two groups during a 6-month period following surgery.Results Compared with the control group,the experimental group had less intraoperative blood loss,shorter hospital stay and shorter operation time,and the differences were statistically significant(all P<0.05).Blood Scr,blood urea nitrogen and serum Cys C decreased after treatment,while urine KIM-1 increased in both groups(P<0.05).The blood Scr,blood urea nitrogen and serum Cys C in the experimental group were lower than those in the control group,while the urine KIM-1 was higher than that of the control group,with statistical significance(all P<0.05).The complication rate of the experimental group was lower than that of the control group,and the difference was statistically significant(t=5.330,P=0.021).Conclusions Compared with renal cyst puncture sclerosis and roof removal,percutaneous nephroscopy 1470 nm laser unroofing has less blood loss,shorter hospital stay,shorter operation time,and lower postoperative recurrence rate.
