摘 要:目的探讨多部门协同质量管理在降低呼吸机相关肺炎发生率中的应用效果。方法组建多部门协同质量管理团队,制定并实施多部门协同质量管理方案,包括呼吸机相关肺炎(ventilator-associated pneumonia,VAP)防控多部门质量监管、多元化培训与考核、VAP防控临床实践标准化。比较多部门协同质量管理前后VAP发生率、VAP防控措施落实率及护理人员气道管理相关技能考核成绩。结果多部门协同质量管理前后呼吸机相关肺炎发生率分别为1.40‰、0.00‰,差异无统计学意义(P>0.05)。VAP防控措施中手卫生,呼吸机回路管理,预防误吸、反流,早期康复干预4项措施落实率明显提高,差异有统计学意义(P<0.05)。重症监护病房护理人员气道管理相关技能考核成绩较管理前明显提高,差异有统计学意义(P<0.05)。结论多部门协同质量管理有助于减少呼吸机相关肺炎的发生,提高护理人员气道管理能力。Objective To explore the application effect of multi-department collaborative quality management in reducing the incidence of ventilator-associated pneumonia.Methods A multi-department collaborative quality management team was established to develop and implement a multi-department collaborative quality management program,including multi-department quality supervision of ventilator-associated pneumonia prevention and control,diversified training and assessment,and standardization of clinical practice of VAP control.The incidence of VAP,the implementation rate of VAP prevention and control measures and the assessment results of airway management skills of nursing staff were compared before and after multi-department collaborative quality management.Results The incidence of ventilator-associated pneumonia before and after multi-department collaborative quality management was 1.40‰and 0.00‰,respectively,with no statistical significance(P>0.05).The implementation rate of hand hygiene,ventilator circuit management,prevention of aspiration and reflux,and early rehabilitation intervention were significantly improved in VAP prevention and control measures(P<0.05).The scores of airway management related skills of ICU nurses were significantly improved compared with those before management,and the difference was statistically significant(P<0.05).Conclusions Multi-department collaborative quality management can effectively reduce the incidence of ventilator-associated pneumonia and improve the airway management ability of nurses.
