摘　　要：目的探讨沙库巴曲缬沙坦对自发性高血压大鼠(SHR)心肌钠钙交换体(NCX)的影响。方法将16只雄性SPF级SHR随机分为对照组与观察组,每组8只,另将8只SFP级雄性Wistar健康大鼠作为正常组。观察组给予沙库巴曲缬沙坦治疗,对照组、正常组均给予等容积生理盐水。比较各组大鼠左心室短轴收缩率(LVFS)、收缩末期室间隔厚度(IVSs)、心脏重量指数、心肌NCX蛋白表达水平,以及病理学染色情况。结果干预前,对照组、观察组大鼠IVSs高于正常组,而LVFS低于正常组,差异有统计学意义(P<0.05)。干预前,对照组与观察组大鼠IVSs、LVFS比较,差异无统计学意义(P>0.05)。干预后,3组大鼠IVSs、LVFS两两比较,差异有统计学意义(P<0.05)。观察组大鼠干预后IVSs低于干预前,差异有统计学意义(P<0.05)。对照组大鼠心脏重量指数[(4.25±0.35)mg/g]明显高于正常组心脏重量指数[(3.32±0.25)mg/g],而观察组大鼠心脏重量指数[(3.66±0.25)mg/g]明显低于对照组心脏重量指数,差异有统计学意义(P<0.05)。对照组大鼠蓝色区域明显增多,胶原纤维沉积为(12.42±0.66)%,显著高于正常组的(3.28±0.26)%,差异有统计学意义(P<0.05)。观察组大鼠蓝色区域显著减少,胶原纤维沉积为(5.17±0.58)%,显著小于对照组的(12.42±0.66)%,差异有统计学意义(P<0.05)。对照组、观察组、正常组大鼠心肌NCX蛋白相对表达水平分别为(0.96±0.14)、(0.77±0.16)、(0.56±0.13),3组两两比较,差异有统计学意义(P<0.05)。结论沙库巴曲缬沙坦可抑制SHR心室重构,降低胶原纤维沉积,其作用机制可能与其下调NCX有关。Objective To investigate the effect of sacubitril valsartan on myocardial sodium-calcium exchanger(NCX)in spontaneously hypertensive rats(SHR).Methods A total of 16 male specific pathogen free(SPF)SHRs were randomly divided into the control group and the observation group,with 8 in each group and 8 healthy male SPF Wistar rats were selected as the normal group.The observation group was treated with sacubitril valsartan,and the control group and normal group were treated with equal volume of normal saline.The left ventricular short axis systolic rate(LVFS),end systolic interventricular septal thickness(IVSs),heart weight index,myocardial NCX protein expression level and pathological staining were compared among all groups.Results Before intervention,the IVSs in the control group and the observation group was higher than that in the normal group,while the LVFS was lower than that in the normal group,the differences were statistically significant(P<0.05).Before intervention,there were no significant differences in IVSs and LVFS between the control group and the observation group(P>0.05).After the intervention,the IVSs and LVFS in the three groups were compared,and the differences were statistically significant(P<0.05).The IVSs in the observation group after intervention was lower than that before intervention,and the difference was statistically significant(P<0.05).The heart weight index of the control group[(4.25±0.35)mg/g]was significantly higher than that of the normal group[(3.32±0.25)mg/g],while the observation group[(3.66±0.25)mg/g]was significantly lower than that of the control group,with statistical significance(P<0.05).The blue area in the control group was significantly increased,and the collagen fiber deposition was(12.42±0.66)%,which was significantly higher than(3.28±0.26)%in the normal group,and the difference was statistically significant(P<0.05).The blue area in the observation group was significantly reduced,and the collagen fiber deposition was(5.17±0.58)%,which was significantly less than(1
