摘　　要：目的了解那曲市班戈县藏族成人甲状腺激素水平,分析甲状腺激素参考值范围,为后续甲状腺疾病诊断提供依据。方法按照美国国家临床生化学院(National Academy of Clinical Biochemistry,NACB)检验医学应用准则筛选要求,选取193例班戈县藏族成人作为调查对象,其中112例来自2021年5月至2022年8月班戈县人民医院门诊体检者,81例来自2022年6-8月参加下乡体检者。采用雅培i2000SR化学发光免疫分析仪检测血清促甲状腺激素(thyroid stimulating hormone,TSH)、总甲状腺素(total thyroxine,TT_(4))、总三碘甲腺原氨酸(total triiodothyronine,TT_(3))、游离甲状腺素(free thyroxine,FT_(4))、游离三碘甲腺原氨酸(free triiodothyronine,FT_(3))水平,建立甲状腺激素参考值范围,分析甲状腺激素水平与年龄的相关性及不同乡镇间差异性。结果那曲市班戈县藏族成人甲状腺激素参考值范围分析结果显示,TSH为0.53~6.16μU/ml,TT_(3)为1.07~2.28 nmol/L,FT_(3)为3.30~5.86 pmol/L,TT_(4)为60.68~129.96 nmol/L,FT_(4)为9.74~16.29 pmol/L。血清TSH水平与年龄呈正相关(r=0.16,P=0.026),血清TT_(3)、FT_(3)、TT_(4)、FT_(4)水平与年龄均呈负相关(r=-0.21、-0.23、-0.16、-0.26,均P<0.05)。班戈县5个乡镇间血清TT_(3)、FT_(3)、TT_(4)水平比较,差异均有统计学意义(均P<0.05),其中,保吉乡血清TT_(4)水平低于其他4个乡镇,德庆镇血清FT_(3)水平高于其他4个乡镇(均P<0.05)。结论成功建立那曲市班戈县藏族成人甲状腺激素参考值范围。建议根据年龄分段建立甲状腺激素参考值范围,并在不同乡镇间开展更为深入的碘营养状况及甲状腺功能调查研究。Objective To conduct a survey and study on thyroid hormone levels in Tibetan adults in Baingoin County,Nagqu City,analyse reference range of thyroid hormones,and provide a basis for subsequent diagnosis of thyroid diseases.Methods According to the screening requirements of the National Academy of Clinical Biochemistry(NACB)Laboratory Medicine Application Guidelines,193 local Tibetan adults in Baingoin County were selected as the survey subjects.Among them,112 adults underwent physical examinations at the outpatient department of Baingoin County People's Hospital from May 2021 to August 2022,and 81 adults participated in rural physical examinations from June to August 2022.Using the Abbott i2000SR chemiluminescence immunoassay analyzer,the levels of thyroid stimulating hormone(TSH),total thyroxine(TT_(4)),total triiodothyronine(TT_(3)),free thyroxine(FT_(4)),and free triiodothyronine(FT_(3))were determined.The reference range of thyroid hormones for local residents was established,and the correlation between thyroid hormone levels and age,as well as the differences between different townships were analyzed.Results A reference range for thyroid hormones in Tibetan adults in Baingoin County,Nagqu City had been established(TSH:0.53-6.16μU/ml,TT_(3):1.07-2.28 nmol/L,FT_(3):3.30-5.86 pmol/L,TT_(4):60.68-129.96 nmol/L,and FT_(4):9.74-16.29 pmol/L).TSH level was positively correlated with age(r=0.16,P=0.026),while TT_(3),FT_(3),TT_(4),and FT_(4) levels were negatively correlated with age(r=-0.21,-0.23,-0.16,-0.26,P<0.05).The comparison of TT_(3),FT_(3),TT_(4) levels among 5 townships in Baingoin County showed statistically significant differences(P<0.05).Among them,Baoji Township was lower than the other four townships in the TT_(4) level,and Deqing Township was higher than the other four townships in the FT_(3) level(P<0.05).Conclusions Successfully established a reference range for thyroid hormones in Tibetan adults in Baingoin County,Nagqu City.It is recommended to establish a reference range for thyroid hormones bas
