摘 要:目的探索血清尿酸浓度与老年高血压患者骨质疏松症发生间的潜在关系。方法本研究共纳入了2021年1月—2023年12月就诊于新疆维吾尔自治区人民医院高血压诊疗研究中心的884例高血压患者,根据所有研究对象的血清尿酸水平的三分位数将其等分为3组,<287μmol/L为T1组,287~371μmol/L为T2组,>371μmol/L为T3组。采用限制性立方样条(restricted cubic spline,RCS)评估血清尿酸与骨质疏松症间的剂量反应关系,采用logistic回归模型进一步验证其关联。结果RCS显示尿酸与骨质疏松间存在U型关系,尤其是当尿酸<315μmol/L或>380μmol/L时,其骨质疏松症的患病风险明显增加。另外,logistic回归模型进一步验证,与T2组相比,T1组和T3组患骨质疏松症的风险较高(P<0.05)。结论老年高血压患者的血清尿酸水平与骨质疏松症密切相关,且两者存在U型关系。Objective The aim of this study was to explore the potential relationship between serum uric acid concentration and the development of osteoporosis in elderly hypertensive patients.Methods A total of 884 hypertensive patients admitted to the Hypertension Center of the People's Hospital of Xinjiang Uygur Autonomous Region from January 2021 to December 2023 were included in this study.All participants were divided into three groups according to the tertiles of serum uric acid levels,<287μmol/L for T1 group,287-371μmol/L for T2 group,>371μmol/L for T3 group.Restricted cubic spline(RCS)was used to evaluate the dose-response relationship between serum uric acid and osteoporosis,and logistic regression model was used to further verify the association.Results The RCS showed a U-shaped relationship between uric acid and osteoporosis,especially when uric acid was less than 315μmol/L or more than 380μmol/L,the risk of osteoporosis was significantly increased.In addition,the association was further validated by multi-model logistic regression,which showed a significantly higher risk of osteoporosis in the T1 and T3 groups compared to the T2 group(P<0.05).Conclusion Serum uric acid is closely related to osteoporosis in elderly patients with hypertension,and there is an obvious U-shaped relationship between the two.
