摘　　要：新生儿呼吸窘迫综合征(RDS)是新生儿呼吸窘迫的常见原因。产前皮质激素(ACS)、新生儿肺表面活性物质(PS)替代疗法和新生儿高级呼吸管理等干预策略改善了RDS患儿的预后,但RDS仍然是早产儿发病和死亡的主要原因之一。为了有利于管理新生儿RDS,世界多国发布了临床管理指南/专家共识,但这些指南/共识并没有达成一致意见。本文在简要回顾Avery ME等人的研究成果、了解发现和认识RDS的漫长过程、熟悉诊治RDS的逐渐演变的基础上,强调RDS的病因学和分类、评估与诊断,结合多国指南/共识,突出最新证据,并对美国RDS新生儿专家工作组(RDS-NExT)共识陈述作简要介绍和评价。Neonatal respiratory distress syndrome(RDS)is a common cause of neonatal respiratory distress.Although intervention strategies such as prenatal corticosteroids(ACS),neonatal pulmonary surfactant(PS)replacement therapy,and advanced neonatal respiratory management have improved the prognosis of neonates with RDS,it remains one of the leading causes of morbidity and mortality in premature infants.In order to facilitate the management of neonatal RDS,multiple countries around the world have issued clinical management guidelines and expert consensuses,but there has been no consensus on these guidelines or consensuses.By giving a brief review of Avery ME and others’research results and introducing the long process of discovering and recognizing RDS as well as the gradual evolution of diagnosis and treatment of RDS,this article emphasizes the etiology,classification,evaluation,and diagnosis of RDS,highlights the latest evidence based on multinational guidelines/consensuses,and provides a brief introduction and evaluation of the consensus statement of the RDS-Neonatal Expert Taskforce(RDS-NExT)in the United States.
