摘　　要：目的探讨外周血中白细胞介素、肿瘤坏死因子及血清铁蛋白(serum ferritin,SF)对老年支原体肺炎(MPP)严重程度的预测价值。方法选取2022年10月至2023年10月入住江苏省省级机关医院的86例老年MPP病人为MPP组,选择同期100例健康老年人作为对照组,检测受试者血清IL-6、IL-10、IL-17、TNF-α、SF、WBC及CRP水平,比较老年MPP轻症、重症组及对照组血清中上述指标的差异,探讨其预测MPP严重程度的价值。结果与对照组相比,MPP病人血清IL-6、IL-10、IL-17、TNF-α及SF水平明显升高(P<0.05),MPP重症组升高更显著(P<0.05)。各组间WBC、CRP水平差异无统计学意义。IL-6、IL-10、IL-17、TNF-α及SF预测老年重症MPP的AUC分别为0.64、0.80、0.89、0.71、0.80。结论老年MPP病人血清IL-6、IL-10、IL-17、TNF-α及SF水平显著提高,可作为预测老年重症MPP的有效生物标志物。Objective To investigate the predictive value of interleukin,tumor necrosis factorα(TNF-α)and serum ferritin(SF)in peripheral blood for the severity degree of mycoplasma pneumoniae pneumonia(MPP)in the elderly.Methods A total of 86 elderly patients with MPP admitted to Jiangsu Province Official Hospital from October 2022 to October 2023 were enrolled in the MPP group,and 100 healthy individuals were enrolled in the control group during the same period of time.The levels of serum interleukin 6(IL-6),IL-10,IL-17,TNF-α,SF,white blood cell(WBC)and C-reactive protein(CRP)were detected and compared among the control group and the mild and severe MPP groups,and the value in predicting the severity of MPP in the elderly was expored.Results The serum levels of IL-6,IL-10,IL-17,TNF-αand SF were significantly higher in the MPP subgroups than those in the control group(P<0.05),especially in the severe MPP group(P<0.05).There were no statistically significant differences in WBC and CRP between the groups.The areas under receiver operating characteristic curve of IL-6,IL-10,IL-17,TNF-αand SF for predicting severe MPP in the elderly were 0.64,0.80,0.89,0.71,and 0.80,respectively.Conclusions Serum levels of IL-6,IL-10,IL-17,TNF-αand SF are significantly elevated in the elderly patients with MPP,and can be used as effective biomarkers to predict severe MPP in the elderly.
