摘　　要：目的:比较视黄醇结合蛋白4 (RBP4)、白细胞介素6 (IL-6)在不同射血分数心力衰竭患者中的表达及临床意义。方法:选取2023年1-9月石河子大学第一附属医院住院的150例心衰患者作为研究对象,分为射血分数降保留型心力衰竭(HFpEF)组、射血分数中间型心衰(HFmrEF)组、射血分数降低型心衰(HFpEF)组,每组各50例,在同期住院患者中选取50例心功能正常但患有其他心血管疾病的患者作为对照组。收集入选患者的一般资料及实验室检查,应用酶联免疫吸附法检测血清RBP4及IL-6的水平。结果:与对照组比较,心衰患者的RBP4、IL-6表达量明显升高,差异均有统计学意义(H=62.87、59.77,P<0.01)。logistic回归显示冠状动脉粥样硬化性心脏病、糖尿病、心房颤动病史及患者的血清RBP4、IL-6、尿素氮、空腹血糖、血清胱抑素-C (Cys-C)是心力衰竭患者的危险因素;多因素回归分析显示不同射血分数心力衰竭患者的危险因素包括RBP4和IL-6;受试者工作特征(ROC)曲线分析显示,RBP4诊断心力衰竭的灵敏度为0.613,特异度为0.957。结论:血清RBP4、IL-6是心力衰竭患者的危险因素,RBP4对心衰患者的临床诊断具有一定的参考价值。Objective:To compare the expression and clinical significance of retinol RBP4 and IL-6 in heart failure patients with different ejection fraction.Methods:A total of 150 patients with heart failure were selected,there were 50 cases in HFrEF group,HFmrEF group and HFpEF group.50 patients with normal cardiac function and other cardiovascular diseases were selected as control group.The general clinical data and laboratory tests of the selected patients were collected.The serum levels of RBP4 and IL-6 were detected by enzyme-linked immunosorbent assay.Results:Compared with the control group,the levels of RBP4 and IL-6 in patients with heart failure were significantly increased(H=62.87,59.77;P<0.001).Logistic regression analysis showed that coronary heart disease,diabetes mellitus,atrial fibrillation,RBP4,IL-6,urea nitrogen,fasting blood glucose and Cystatin-C were the risk factors of heart failure.Multivariate regression analysis showed that RBP4 and IL-6 were risk factors for heart failure patients with different ejection fractions.ROC curve analysis showed that the sensitivity and specificity of RBP4 in the diagnosis of heart failure were 0.613 and 0.957.Conclusion:Serum RBP4 and IL-6 are risk factors for patients with heart failure,and RBP4 has a certain reference value for the clinical diagnosis of patients with heart failure.
