子宫肌瘤剥除术联合来曲唑治疗子宫肌瘤的临床观察

摘　　要：目的:对比腹腔镜下子宫肌瘤剥除术联合术后来曲唑治疗子宫肌瘤的临床观察及对血清雌激素(E_(2))和雌激素受体(ER)水平的影响。方法:选取2020年1月—2022年12月河南省伊川县人民医院收治的76例子宫肌瘤患者作为研究对象,通过随机数表法将患者分为对照组(n=38)和研究组(n=38)。对照组与研究组均进行腹腔镜下子宫肌瘤剥除术,而研究组术后第2 d开始口服来曲唑治疗,每日1次,每次2.5 mg,连续3个月。通过比较两组患者术前以及术后1个月后血清中E_(2)、ER的水平,患者术后1个月、6个月、12个月的子宫肌瘤的复发情况以及在术后12个月内的不良反应发生率情况。结果:术后1个月,研究组E_(2)以及ER水平均显著低于对照组,差异均有统计学意义(t=10.831、11.168,P<0.05);术后,研究组子宫肌瘤的总复发率显著低于对照组,差异有统计学意义(χ^(2)=5.208,P<0.05);术后12个月内,研究组不良反应总发生率显著低于对照组,差异有统计学意义(χ^(2)=4.547,P<0.05)。结论:通过腹腔镜下子宫肌瘤剥除术联合术后来曲唑治疗子宫肌瘤的联合治疗方案,为患者提供了全面且个体化的治疗方案,提高了治疗效果和患者生活质量。Objective:To compare the clinical observation of laparoscopic uterine fibroid combined with postoperative trazole in the treatment of uterine fibroid and its influence on serum E2 and ER levels.Methods:76 female patients with uterine fibroid who were treated in the hospital from January 2020 to December 2022 were collected as research objects,and they were divided into control group(n=38)and study group(n=38)by random number table method.Both the control group and the study group received laparoscopic uterine fibroid removal,while the study group took letrozole orally the second day after the operation,once a day,2.5 mg each time,for three months.By comparing the serum E2 and ER levels of the two groups before and 1 month after operation,the recurrence of uterine fibroid 1 month,6 months and 12 months after the operation and the incidence of adverse reactions within 12 months after the operation were compared.Results:1 month after surgery,E2 and ER levels in the study group were significantly lower than those in the control group(t=10.831,11.168;P<0.05).After surgery,the total recurrence rate of uterine fibroids in the study group was significantly lower than that in the control group(χ^(2)=5.208,P<0.05).Within 12 months after operation,the total incidence of adverse reactions,including vomiting,nausea,lower abdominal pain,fatigue and anal distention in the study group was significantly lower than that in the control group,with statistically significant difference(χ^(2)=4.547,P<0.05).Conclusion:The combined treatment scheme of laparoscopic uterine fibroid and subsequent trazole for Uterine fibroid has provided patients with a more comprehensive and individualized treatment scheme,and improves the treatment effect and quality of life.
