3种不同术式治疗下肢静脉曲张的回顾性研究

摘　　要：目的:回顾性研究3种不同术式治疗下肢静脉曲张的疗效。方法:选取2018年6月—2023年5月焦作市人民医院收治的88例下肢静脉曲张患者作为研究对象,根据治疗方法将患者分为内热消融术组(35例),高位结扎组(27例)和传统剥脱术组(26例)。内热消融术组患者采用静脉内热消融术进行治疗,高位结扎组患者采用大隐静脉高位结扎进行治疗,传统剥脱术组患者采用传统剥脱术进行治疗。比较3组患者的手术时长、术中出血量、住院时间、治疗有效率、视觉模拟量表(VAS)评分和淤积性皮炎、浅静脉炎、隐神经受损等不良反应发生率。结果:术后7 d,内热消融术组、高位结扎组患者治疗有效率高于传统剥脱术组,差异有统计学意义(χ^(2)=8.748,P<0.05);内热消融术组患者手术时长、住院时间均短于高位结扎组、传统剥脱术组,术中出血量少于高位结扎组、传统剥脱术组,差异有统计学意义(F=13.34,P<0.05);术后24 h、术后3 d,内热消融术组患者VAS评分低于高位结扎组、传统剥脱术组,差异有统计学意义(F=1.02,P<0.05);术后,内热消融术组、高位结扎组患者淤积性皮炎、浅静脉炎、隐神经受损并发症发生率低于传统剥脱术组,差异有统计学意义(χ^(2)=7.441,P<0.05)。结论:静脉内热消融术和大隐静脉高位结扎治疗下肢静脉曲张患者能够提高治疗有效率,并且降低术后并发症发生率,静脉内热消融术能够减轻患者术后疼痛,促进患者术后恢复。Objective:To explore the retrospective study of three surgical methods in the treatment of varicose vein of lower limb.Methods:A total of 88 patients with varicose vein of lower limb in the hospital were enrolled from June 2018 to May 2023.According to different treatment methods,they were divided into thermal ablation group(35 cases,intravenous thermal ablation),high ligation group(27 cases,high ligation of great saphenous vein)and traditional stripping group(26 cases,traditional stripping).Patients in the internal heat ablation group were treated with intravenous heat ablation,patients in the high ligation group were treated with high ligation of the great saphenous vein,and patients in the traditional exfoliation group were treated with traditional exfoliation.The operation time,intraoperative blood loss and hospitalization time,response rate of treatment,scores of visual analogue scale(VAS)and the incidence of adverse reactions(stasis dermatitis,superficial phlebitis,saphenous nerve damage)after surgery were compared among the three groups.Results:7 days after surgery,the effective rate of internal heat ablation group and high ligation group was higher than that of traditional exfoliation group,and the difference was statistically significant(χ^(2)=8.748,P<0.05).The operation duration and hospital stay in the internal heat ablation group were shorter than those in the high ligation group and the traditional exfoliation group,and the intraoperative blood loss was less than that in the high ligation group and the traditional exfoliation group.The difference was statistically significant(F=13.34,P<0.05).24 h and 3 d after surgery,VAS scores in the internal heat ablation group were lower than those in the high ligation group and the traditional exfoliation group,with statistically significant difference(F=1.02,P<0.05).After surgery,the incidence of complications of stasis dermatitis,superficial phlebitis and saphenous nerve damage in the internal heat ablation group and high ligation group was lower than that in
