摘　　要：目的:探究血清胱抑素C (CysC)、同型半胱氨酸(Hcy)及低密度脂蛋白(LDL)在急性脑梗死(ACI)后偏瘫患者中的表达水平,分析其对ACI偏瘫患者预后的评估价值。方法:选取2021年4月—2023年5月雷州市人民医院收治的100例ACI偏瘫患者以及进行健康体检的50例正常体检者作为研究对象,将ACI偏瘫患者设为观察组,正常体检者设为对照组。比较两组受试者的一般临床资料及血清CysC、Hcy、LDL水平,并采用logistic回归分析及绘制受试者工作特征(ROC)曲线确定其对ACI患者预后的预测价值。结果:观察组患者血清CysC、Hcy及LDL明显高于对照组,差异均有统计学意义(t=8.88、22.69、4.346,P<0.05);logistic回归分析结果显示,血清CysC、Hcy及LDL水平是影响ACI偏瘫患者发生不良预后的独立危险因素;ROC曲线分析显示,CysC、Hcy及LDL水平三项联合预测ACI患者不良预后的价值最高,其最佳截断值为1.21 mg/L,21.52μmol/L,3.13 mmol/L。结论:血清CysC、Hcy和LDL与ACI偏瘫患者的预后具有相关性,三项联合检测对于评估患者的预后有重要价值。Objective:To explore the expression levels of serum cystatin C(Cys C),homocysteine(Hcy)and low-density lipoprotein(LDL)of hemiplegic patients after acute cerebral infarction(ACI),and analyze their prognostic value of ACI hemiplegic patients.Methods:A total of 100 patients with ACI hemiplegia admitted to Leizhou People’s Hospital from April 2021 to May 2023,and 50 normal medical examiners who underwent health checkups were selected as the study subjects.Patients with ACI hemiplegia were in the observation group,and normal medical examiners were in the control group.General clinical data,the serum levels of Cys C,Hcy and LDL of the two groups were compared.The predictive value of this method for the prognosis of ACI patients was determined by logistic regression analysis and ROC curve.Results:The levels of serum Cys C,Hcy and LDL were significantly higher in observation group than in control group(t=8.88,22.69,4.346;P<0.05).According to logistic regression analysis,Serum Cys C,Hcy and LDL levels were risk factors for poor prognosis in patients with ACI hemiplegia.ROC curve analysis showed that the combination of CysC,Hcy and LDL levels had the highest value in predicting adverse prognosis in ACI patients,and the optimal cut-off value was 1.21 mg/L,21.52μmol/L and 3.13 mmol/L.Conclusion:Serum Cys C,Hcy and LDL levels are correlated with the prognosis of patients with ACI hemiplegia,and the two combined tests are of great value in evaluating the prognosis of patients.
