“土郁夺之”理论探讨功能性消化不良辨证思路与治疗

摘　　要：目的:基于“土郁夺之”理论出发,探讨功能性消化不良(functional dyspepsia,FD)辨证思路与治疗。方法:基于对“土郁”理论的阐释和FD的现代研究,分析该病的病因病机,总结其治疗原则,并报道医案一则予以佐证。结果:FD的病机为气机的升降失常,通过以运主之和以疏主之调理脾胃气机,通泄肝气,恢复其功能正常运行,改善患者的消化功能。所举医案为“土郁夺之”理论治疗FD 1例,证属肝郁气滞、肝郁乘脾,方选柴胡桂枝龙骨牡蛎汤合甘麦大枣汤治疗,随证加减,效果显著。结论:从“土郁夺之”理论探讨FD的诊治思路具有临床实用意义,为FD的临床治疗拓宽思路与启示。Objective To explore the dialectical treatment of functional dyspepsia.Methods Based on the interpretation of the theory of“soil depression”and the modern study of FD,the etiology and pathogenesis of the disease were analyzed,the treatment principles were summarized,and a medical case was reported to prove it.Results The rise and fall of FD qi machine,by transporting the main and regulating the main of the main,relieve the liver qi,restore its normal operation,and improve the digestive function of the patient.The medical case mentioned was a case of FD theoretical treatment of“soil depression”,the syndrome was liver stagnation and qi stagnation,liver stagnation and spleen,and the keel oyster soup and Ganmai jujube soup for treatment,with the addition and decrease of the syndrome,the effect was remarkable.Conclusion Discuss the idea of FD from the theory of“soil depression”,and broaden the idea and enlightenment for the clinical treatment of FD.
