苗医外治法辨治卒中后肩手综合征的临证经验

摘　　要：肩手综合征作为卒中后常见并发症之一,发病率逐年上升,现临床治疗方法多样,多采用药物、手术、物理康复、心理疗法等以缓解症状、消除疼痛;但现有治法疗效不稳定,且部分费用昂贵,如何应用安全、有效、价格低廉的方法治疗这一病情复杂、具有挑战的难治性疾病,已成为目前亟待解决的问题。吴远华教授应用苗医外治法治疗卒中后肩手综合征经验丰富,其认为毒损脑络,致乱成疾为发病之本,气壅血滞,筋脉不通为发病之根,以祛除诱因、畅通筋脉为治则,应用熏蒸疗法、刮治疗法、推拿、隔药纸火灸、针疗等几种常用且疗效可靠的外治法治疗,达祛毒散邪、平乱通滞、行气散血、柔畅筋脉之效。现就吴远华教授辨治卒中后肩手综合征经验进行概述,以期为苗医外治法治疗卒中后肩手综合征及类似疾病提供参考。Shoulder-hand syndrome(SHS)is one of the common complications after stroke,and its incidence is increasing year by year.At present,there are various clinical treatment methods,including drugs,surgery,physical rehabilitation,and psychotherapy to relieve symptoms and eliminate pain.However,the efficacy of existing treatments is unstable,and some of them are expensive.How to apply safe,effective and inexpensive methods to treat this complex and challenging refractory disease has become a problem of current attention.Professor Wu Yuanhua has rich experience in the treatment of shoulder-hand syndrome after stroke with the external treatment of Miao medicine.He believes that the root of the disease is the disorder caused by toxic damage to the brain collateral,and the obstruction of qi and blood,and the obstruction of tendons and vessels.In order to remove the inducement and smooth the tendons and vessels as the treatment principle,he uses fumigation therapy,scraping therapy,massage,medication-separated paper fire moxibustion,acupuncture therapy and other commonly used external treatment with reliable curative effect.It can dispel poisons and dissipate pathogens,smooth disorder and stagnation,dispel qi and blood,and smooth tendons and vessels.This article summarizes Professor Wu Yuanhua's experience in the treatment of shoulder-hand syndrome after stroke,in order to provide useful reference for the treatment of shoulder-hand syndrome after stroke and similar diseases with Miao medical external treatment.
