基于“肾为胃之关”理论针灸治疗小儿咳嗽变异型哮喘经验

摘　　要：小儿咳嗽变异型哮喘主要表现为长期反复的慢性咳嗽,此类症状容易对患儿的生活质量及心理健康产生不良影响。李俐主任医师认为该病本质为小儿脏腑功能失职引起的人体气机失调,依据《黄帝内经》中提到的“肾者,胃之关也”,提出咳嗽的根源是由于小儿先天禀赋不足,肾气亏虚,肾气亏虚导致胃关开合失调,胃本应通降之气上逆,从而使得肺部肃降之气同胃部上逆之气相冲,引发肺气上逆而咳嗽。治则当从调理肾脏、脾胃入手,其具体治法为温肾纳气,健脾和胃。李俐主任医师针对此病选穴精当,针法独特,补泻结合,其临床疗效及预后可靠。The primary manifestation of pediatric cough variant asthma is a persistent,long-term,chronic cough,which significantly impacts children's quality of life and has numerous negative impacts on mental health.The lead doctor,Li Li,posits that children's illnesses stem from internal organ malfunctions due to imbalances in the body's qi,as stated in the Yellow Emperor's Classic of Internal Medicine:“the kidney,the stomach of the gateway”.He attributes the cough to a lack of innate qi in the child,a deficiency in kidney qi,and kidney qi,resulting in an imbalance in the opening and closing of the stomach pass.He suggests that the stomach functions by descending the qi upwards,leading to the lungs purging qi while the stomach reverses it upwards.When the qi from the lung's purging and descending side meet the qi from the stomach's upward reversal,it causes the lung's qi to rise and cough.Therapy ought to commence by managing the kidneys,spleen,and stomach,focusing specifically on heating the kidneys and qi to fortify both the spleen and stomach.Dr.Li Li,overseeing the selection of suitable acupuncture points,distinctive needle techniques,the mix of tonic and diarrhea,and its clinical effectiveness and prognosis,is dependable.
