两种术式治疗腰椎管狭窄症的疗效比较

摘　　要：目的比较单侧双通道脊柱内镜技术(UBE)与经皮椎间孔镜技术(PTED)治疗腰椎管狭窄症的疗效。方法将91例腰椎管狭窄症患者根据手术方式不同分为UBE组(采用单侧双通道脊柱内镜技术治疗,43例)和PTED组(采用经皮椎间孔镜技术治疗,48例)。比较两组手术情况、疼痛VAS评分、ODI评分、术后并发症发生情况,采用改良MacNab标准评价疗效。结果两组均获得12个月随访。手术时间、术后住院时间两组比较差异均无统计学意义(P>0.05)。术后第3天疼痛VAS评分:两组均低于术前(P<0.05),两组比较差异无统计学意义(P>0.05)。术后6个月ODI评分:两组均低于术前(P<0.05),两组比较差异无统计学意义(P>0.05)。术后并发症发生率、采用改良MacNab标准评价的疗效优良率两组比较差异均无统计学意义(P>0.05)。结论UBE和PTED均是治疗腰椎管狭窄症安全、有效、可靠的微创方法,均可实现腰椎管减压,并获得良好的临床效果。Objective To compare the efficacy of unilateral biportal endoscopy(UBE)and percutaneous transforaminal endoscopic discectomy(PTED)in the treatment of lumbar spinal stenosis.Methods The 91 patients with lumbar spinal stenoses were divided into UBE group(43 cases were treated with UBE)and PTED group(48 cases were with PTED),according to different surgical methods.The surgical status,pain VAS,ODI score and postoperative complications of the two groups were compared,and the modified MacNab standard was used to evaluate the curative effect.Results Both groups were followed up for 12 months.There were no statistical differences in operation time and postoperative hospital stay between the two groups(P>0.05).The pain VAS of 3 d after operation:The two groups were both lower than that before surgery(P<0.05),and there was no statistical difference between the two groups(P>0.05).The ODI score of 6 months after surgery:Two groups were both lower than that before surgery(P<0.05),and there was no statistical difference between the two groups(P>0.05).There were no statistical differences in the incidence of postoperative complications and the excellent-good rate of efficacy evaluated by the modified MacNab standard between the two groups(P>0.05).Conclusions Both UBE and PTED are safe,effective and reliable minimally invasive methods for treatment of lumbar spinal stenosis,and both methods can achieve lumbar spinal canal decompression and good clinical results.
